[Question #6564] New Symptoms

Hello 

I posted a week or so ago about what I thought were prodrome symptoms but what Terri advised were probably just symptoms of my fungul infection on my inner thigh. Since then I have developed some new symptoms. I discovered a red spot on my penis head 17 days post unprotected sex. It was flat, no blisters, no real pain associated with it. It has now been present for 8 days with little change. I’ve also been experiencing slightly more frequent urination and a light itching in the general genital region (could be mental I have anxiety issues). Also when I get an erection the skin that’s normally hidden under skin folds is slightly “sticky” and sensitive to touch. I also noticed small red bumps around the underside of the head of my penis. I can’t decide if they’re new or if they are pearly penile papules. My PCP examined the mark on my penis head and said it was just an abrasion. Is it normal that it hasn’t healed yet in 8 days? Does any of this sound like herpes? I’m almost a full four weeks post exposure at this point but still slightly nervous I’m about to experience an outbreak. Any insight would be greatly appreciated!

The red area that you describe on the head of the penis does not sound at all herpetic.  Also, herpes does not cause increased urinary frequency.  In terms of the mark on the penis head, herpes outbreak would not stick around that long without changing the way it looked - it would evolve into different looking things over the 8 days.  I think if you were going to have a herpes outbreak from this concerning encounter, you would have had it by now.  I don't think you are about to experience an outbreak, honestly.

Thank you again for your helpful response Terri. Now that I am four weeks post exposure would it be worthwhile to get a blood test to quell my fears or would you not recommend doing so? I don't mean to be a bother and I trust you when you say my symptoms don't sound like herpes I just struggle with the coincidence of running into these issues immediately after an unprotected sexual encounter. It's very unlike me to make such an irresponsible decision. Also, concerning my fungul infection. Is it possible that it was sexually transmitted? Again I just struggle with the fact that it occurred post unprotected sex without it being somehow tied together?

Fungal infections can most definitely be sexually transmitted.  No question.  If your partner had a significant amount of yeast in the vagina and vaginal secretions travel around during sex, this could have gotten on your thigh, no question.  Fungal infection that show up as a result of female-male sex most often show up on the shaft of the penis, but I don't think we can rule out this happening on the thigh as well.  However, as you may already know, many men have "jock itch" on the thigh, after more exercising, after hot weather, that kind of thing.  If you want to do an antibody to reassure yourself about your herpes status, you can certainly do that although I don't think it is medically indicated.  But we aren't just physical animals, are we?  We also have minds and we worry.  There is a small chance of a false positive - in my clinic data, it was a 3% chance, but any question about a false positive could be cleared up with the herpes western blot.  Or you could start with the best test, the western blot.  I sense that this may be helpful for you.

Thank you again for such a helpful response. So just one last thing. Now being four full weeks post exposure. In the absence of any genital sores, you would say that any symptoms of light itching, tingling, and soreness in the general genital area I should attribute to either my fungal infection or my anxiety and it would be highly unlikely they are herpes related? Those symptoms don’t indicate herpes with a lack of sores? (Worth noting my previously mentioned abrasion has healed with the use of cortisone cream)

Hey Terri. Just following up. The last few days have been slightly concerning for me so I just wanted to take you again through the past month and get your opinion when evaluating my symptoms as a whole. So I had unprotected sex. I then developed a red rash on my inner thigh. It developed and grew in size and became more inflamed over the next 4-5 days including developed a white dot or two within the red rash. It was diagnosed as a fungul infection and treated with topical ketocozonale which immediately helped and after about two and a half weeks eliminated the rash. 

While on that medicine I began to experience tingling/itching/aching on my buttocks. Weak leg muscles. Fatigue. All of which lasted about a week. 

After that week those symptoms went away. I then developed a small red abrasion on my penis head which lasted about a week. That went away with the use of cortisone cream for about two days. It was examined by multiple doctors who said it didn’t look herpetic. 

After the abrasion went away the buttocks symptoms returned. The aches, tingling, and itching. This time they were paired with some genital itching on the bulge area above my penis (where I shave regularly) as well as small stinging pains in that same area. I also experienced similar stinging pains in my thighs, legs and feet occasionally. Some days are better than others. They’re short pricks and not persistent. 

I have not experienced any concerning lesions, pimples, or skin breaks that I have noticed. I see some red razor bumps where I shaved and a small white skin tag where I shave as well. None of them hurt or burn. Some light itching in the genital area as a whole. And they don’t change over the past four or more days they’ve remained consistent in size and color. They’re not raised. 

Obviously I’m concerned about herpes. But you’ve said these don’t seem like herpes symptoms. Do you still believe that when reading them all in totality like that? And if so do you have any idea what could cause all these symptoms? The most concerning/irritating are the pricks I’m feeling occasionally and the sore/tingling/itching buttocks. From the day I first noticed the rash on my thigh it has been 4.5 weeks. There hasn’t been a day without some sort of symptom under my waste. Would love some advice/insight. 

Also worth noting I’ve been to numerous doctors through urgent care and my PCP none of whom have seemed particularly concerned or stressed. But I feel like they’ve been slightly dismissive. I hope I haven’t broken any rules by posting this follow up or included too many details. Looking forward to hearing your thoughts/advice.

I have reread about your concerns a few more times now and I still am not concerned about herpes here.  You haven't broken any rules, but you are out of questions, I'm afraid.  This just doesn't fit with herpes in my mind, I don't even have slight concerns here.  Again, I have no idea if you might have herpes due to some previous encounter or experience, but THESE symptoms do not concern me.
