[Question #6570] HSV 1 genital and transmission

History:  11/1/12 - I'm a female and my new partner (male) gave oral sex with history of cold sore (unkown to me at the time)
2/12/13  - tested negative for HSV 1 and HSV 2 by real time PCR and IGG 
11/6/13 (9 mos later) tested negative for HSV 1 and HSV 2 by IGG but POSITIVE for HSV 1 by real time PCR
12/13/13  (10 mos after initial test) tested negative for HSV 1 and HSV 2 by IGG
2/10/14 (one year after 1st test) tested negative for HSV 1 and HSV 2 by IGG
I was with this partner for 7 years

new partner 12/23/19 - 7 years after first test - (now I'm concerned about passing HSV 1 to him since I have had one diagnosis on 11/6/13 by real time PCR), 
12/23/19 tested positive for HSV 1 (47.80) by IGG but NEGATIVE for real time PCR

I am so very confused.  What is the likelihood that I will pass HSV 1 genitally  or orally to my new male partner via oral sex or regular sex?  The diagnosis is so confusing?

When you say that you had a real time positive PCR, was that a swab from an active lesion and was that lesion genital?  I need this information to  help answer your question most accurately, please.  You clearly have HSV from your antibody test but the location of your infection,  I am unclear about.

Yes it was a swab from an active genital lesion.  I am mostly concerned as to now how can I be spreading it.  Will my partner possibly get it from me on his genitals if we have genital sex or will he get it from me orally if we have oral sex?

HSV 1 genital infection is very rarely transmitted via intercourse (it is possible but highly unlikely) because HSV 1 genital infection is rarely active in the genital area.  After a person has been infected with HSV 1 genital infection for 2 years, the average amount of viral shedding is 4 days per year.  Also, if your partner has ever had a cold sore, they are already infected and would be highly unlikely to get this in a new location on their body. 
You don't mention (unless I missed it) that you have oral outbreaks as well - do you?  It sounded like you only had this genitally and if that is the case, then kissing is not a risk.

Thank you so very much!  You have relieved my fears greatly!  You are correct, I have not had oral lesions, only genitally.  One more question:  So if I do ever have another genital lesion and we have genital sex, can he get it genitally as well or only if he gives me oral sex could he get it orally since it is HSV1?

The chances that he could acquire this are very remote but not impossible.  the risk is about the same for having intercourse or him giving you oral sex - very low.
