Hello doctor.
Im married guy from israel who trying to do only safe things when its outside my marriage (not proud about it and trying to stop with this.) All im doing is massages with handjob at the end and in few cases protected oral sex. Before 2 days i had a different event. I engaged with Russian csw in tel aviv israel (she's tourist and russian citizen) she gave me massage and fingered my anus for few minutes, she gave me unprotected oral and i licked her anus for maybe 2 minutes. I didn't saw ant sores or blood in her anus, but i have some small cut in my mouth. Im really afraid about hiv, i know about other risks. I don't know her hiv status but i asked her to do only safe things.
Today i talked with a local hiv clinic and they told me i have nothing to worry about and they not recomend any testing. But the guy who talked to me isn't a doctor so im not sure about it. I will be gald to hear you're opinion.
Do you think it is possible to catch hiv from event like this?
Is it safe to do sex with my wife.
Do i need to wait 4 weeks and test for hiv?

I agree exactly with the advice you received at your local HIV clinic. First, even though HIV is relatively common in Russia, often acquired heterosexually or by shared injection equipment, it is also true that the very large majority of sexually active Russian women don't have it. And even if she did, there has never been a proved case of HIV transmission oral to penis. (A few men over the years have suspected that's how they were infected, but virtually all of those were men having sex with other men.) Hand-genital contact is risk free, and there also are no reported cases of transmission by analingus (oral-anal sex). By far your highest risk from the activities described is a gastrointestinal infection, if your partner happened to be carrying a GI virus, for example. Be on the alert for the next 1-2 weeks for illnesses with nausea, vomiting, or diarrhea.
For those reasons, I see no need for HIV testing or for you to discontinue unprotected sex with your wife. (If I personalize things, if somehow I were in your situation, I woudln't be tested and would continue sex with my wife.)

Just to be clear, you mentioned rates of hiv among russian woman's. She is A sex worker, so i guess the rates are higher among woman's like her, i dont think she's a drug user. Is it change something?

Thanks for clarifying your partner's status. It isn't valid to consider all female sex workers as high risk. Escorts (expensive, booked by appointment, etc) tend to know the score, care about their health, use condoms or otherwise have safe sex only (as in your case), and get tested frequently. They also tend to have low risk clients -- men like you, e.g. married etc. All in contrast to most brothel workers, street walkers, bar pick-ups, etc. I'm not positive how these predictors sort out in Russia, but I suspect they're not much different than elsewhere. None of this guarantees your partner didn't have HIV, but as I said, even if she did, the exposures described carried little or no risk for HIV transmisison.---

Ok thank you. When saying little risk for hiv, do you mean it's sense or logical way for hiv or it's more theoretical risk. Did you see or heard about man or woman that catch hiv by rimming or receiving oral, fingers? Because for me there's a lot of difference between theoretical risk or low risk. I'm trying to do only no risk thing. I know theoretical almost everything can be possible, but low it's not theoretical.
Sorry for been snoozed. But im still nervous.

I don't understand what you mean by "it's sense or logical way for hiv or it's more theoretical risk". In my 45 years in the STD business, including the 35 since HIV/AIDS appeared, I have never seen or even heard of a case of HIV acquired by rimming, by oral to penile transmission, or by fingering.
As for low vs theoretical risk, I disagree with you. There has never been a known case of HIV transmitted oral to penis. But I believe it can occur. Whether you call that "low" or "theoretical" is purely in your mind; there's obviously no important difference.
Above I said you do not need to be tested for HIV. From a medical standpoint, that remains true. However, I recommend you be tested anyway. It is obvious that my reasoned, science-based advice is not helping you understand and believe you were not infected. I hope a negative test result will achieve that goal. Be assured that I suggest testing strictly for reassurance, not because you are at risk. You are not.
That completes the two follow-up comments and replies included with each question and so ends this thread. If you remain concerned, I suggest you carefully re-read the entire discussion. But pelase don't return to ask the same questions again. There's nothing more we can add. OIK?
