[Question #6743] Urgent Testical Pain

Hello Doctors.

on Feb 3 2020 I had unprotected oral sex performed on me by a TS sex Worker.  No other interaction.   I have had no symptoms for 30+ days until last night when I noticed my left testical had significant pain when touched and feels like the tubes inside are swollen.  I have had unprotected sex with my girlfriend 4-5 times since the event one month ago.

In review on line I see that the main cause appears to be  Epididymitis caused by gonorrhea. There is no doubt that this is a real pain and not in my head.  I went to ER, because I could not get in with a Urologists' for at least 40 days.  They took urine sample and ultrasound.  Doc said he assumed it is an infection.  Said they saw a couple Hydroseals but doesn't think that would cause the pain.  But test won't be back for several days.  I still have no other symptoms.

My question is, it is most likely that I have Gonorrhea with no other symptoms? The pain seemed to come all of a sudden, like from no pain to 24 hours being significant pain when manipulated.  


Could my girlfriend and I still have no Symptoms after having sex off and on for past month (unprotected) if I had an STI?


 I am so worried that I gave this to my GF.  

Please help.

Welcome to the Forum.  I'll be glad to comment.  There are a number of possible causes of the discomfort that you are experiencing.  While these include epididymitis as a complication of a urethral STI, other causes include urinary tract infections causing non-STI epididymitis (more common in men over age 40), and kidney stones as well as STIs.  When men have STI-related urethritis, the causes are typically gonorrhea or chlamydia and a penile discharge is present.  For you to not have had symptoms for more than a month since your receipt of oral sex make STIs, and gonorrhea in particular, unlikely.  Perhaps you could provide some additional information which may help clarify things:

How old are you?

Is there any burning on urination?

Is the pain there continuously and has it changed since it began?

What urine tests were performed?  I presume that your urine was tested for: A. gonorrhea and chlamydia (results should be available in 2-3 days, B. bacterial causing non-STI urinary tract infection (culture results may take 3-4 days), and C. a urinalysis which should have provided results while you were in the emergency department.  Do you know the results of the urinalysis?

Were you provided antibiotics?  If so, which ones.

Thank you so much for your prompt reply. I am 48 years old. 

They did not tell me any results of iron other than it wasn’t cloudy and or they could rule out kidney stones. 

There is no burning with urination. 

Pain it there continuously but really only hurts when it is touched or clothes bind up in the crotch area. 

The gave me antibiotics Doxcy but I have yet to take them 

The pain is no worse than it was this morning , but I feel like I am getting pain in lower back and abdominal area. But not sure if that is now anxiety. 

Other than the g/c I don’t know what if any tests were performed. I was not given a result of urninalisys. 

Is there anything else I can provide.? Since reading I am convinced that the only cause can be gonorrhea!  I can’t find any other reason. But with the time and the lack of other symptoms I am praying it is not!!

Thanks for the additional information.  Your age places you in the age range where IF this is an infection, it is quite possible that this is related to a urinary tract infection and or prostate infection.  Not having evidence of kidney stones is also helpful.  Your low back and abdominal discomfort could also be related. 

I am doubtful that this is gonorrhea. Chlamydia is a possibility but so is prostate infection and a related UTI.  I would suggest the following:

1.I would call the emergency room and find out the results of your urinalysis as well as what tests were sent.

2.   If you have a regular doctor you should see him/her and, if the following tests were not done, be sure you have a clean catch mid-stream urine culture as well as tests for gonorrhea and chlamydia.  Your regular doctor should be able to have you see a urologist today or tomorrow. If you don't have a regular doctor, I would call around to other doctors- urologists or general internists and get seen quickly.

3.  I would go on and start to take the antibiotics.  If possible, get the tests I mention above before taking them as they will change the culture test results.

I hope this helps.  While I cannot make a diagnosis over the internet and that is not the purpose of this forum, my advice is to be seen sooner than later.  I suspect you do need treatment but the tests will help you to know what treatment is needed as well as whether your partner may be at risk from STI exposure.  It is not sure that she has been exposed and the tests will help.  EWH

Thank you doctor.

So I am clear,  you said to make sure "these tests are done"  I am not sure if you were to put in a test you wanted other than the G?C test which has been done yesterday.

Also you mentioned Chlamydia, as a causes, is that possible from this single exposure oral?  I thought that was exceedingly rare?

My wife has been having significant pain with her period as of last night.  Would that be an indication of infection?

I neglected to mention that I had a vasectamy 3 years ago, not sure that would play a factor.

I am getting the sense that you believe that I may have an STI (I know you can not diagnose)  but is that your feeling?  That I could have had no symptoms of puss, painfull urination and still be Epiditymits from STI?

The pain today is slightly less today, but still there,  I can touch and move it around more with out the sharp pain that was there yesterday.  

I know the only way to be sure is to wait for the G/C test results but that will be at least 2 more days.  I am freaking out that I may have given this to my girlfriend.  

Can you please clarify more for me?

I'm afraid you misunderstood my earlier message.  There is much to suggest this is NOT an STI and that if it is an STI it is NOT GC.  If your ER doctor did not do the tests I suggested, he should have and you should get them now these tests include:

1.  Chlamydia as well as gonorrhea (typically current tests automatically test for both organisms on a single specimen.  FYI, in the lab, results take less than 6 hours to be available.

2.  The test that showed that this is not kidney stones was most probably a urinalysis.  It should have additional information such as the presence of white blood cells, bacteria, etc.

3.  A urine culture.  As I said, this could well be non-STI epididymitis due to urinary tract bacteria related to a UTI and/or prostatitis.

Instead of generating more self perpetuating anxiety, see a doctor and if these tests have not been done, get them done. If they have been done, start the antibiotics and wait for test results.

Sorry one last bit of clarification.  I know 3 is the limit and I will not ask anything further.

What other possible STI could it be from my exposure?  So it is my understandning you would anticipate Negative Gonnorhea and Chlamydia's Test results?

While acquisition of chlamydia from receipt of oral sex is relatively uncommon, it does happen and it is the major other STI that causes STI-related epididymitis.  As I have said on several occasions above, a number of things suggest that what you are experiencing is NOT an STI including your age, the long period between your exposure and the onset of your scrotal discomfort and the lack of STI signs, amongst others.  That is the reason I suggested other tests (i.e. a urine culture).  EWH

Oh, I understand now.  I used the wrong Abbreviation.  I assumed G/C was an abbreviation for Gonorrhea and chlamydia.  So I assumed your response was to both.  I now see it was to only Gonorrhea.  

So I am screwed.  Chlamydia seems most logical.  And with the pain my girlfriends is having it seems that is the cause.
