[Question #6891] Worry about HIV, pls Dr. HHH

Good evening Mister Handsfield

First of all respect for work & thank you for the service.

To my situation. I am female 30 y old living in Switzerland.

In march 2, 2020 I had protected encounter (vaginal) but unprotected oral sex (blowjob) No semen, but I did some deepthroat.

I am really stressed now bc I got a small cold, I can not say when it started, or if I had it already when I met him.

What I am stressed are my lymph nodes and my muscle aches on my thighs/ burning sensation under my arms/ chest.

My tests:

28 days Combo Alere Det. Rapid Negativ

42 days Combo alere determine rapid negativ

60 days combo Ag+Ak lab (no rapid) = negativ

Lues/ gonnorra/ chlamydia neg.

I wish I could feel releaved but my nodes and burning sensation under my arm/ thighs still hurt. I noticed a small lymph node on my left groin and it makes me uncomfortable. Sometimes I can not sleep on my stomach.

I went to the Checkpoint for hiv/ aids & I showed him my nodes/ pain.

He said, they are very small & you can move them. About my burning sensation he sais it is my muscle on my thigh and my stress. He is confident that I am HIV -.
After he touched my nodes under my armpit hours later it burnt like hell under my arm/ my ears got warm and also a side of my neck hurted..

I would really appreciate your assessment and if there is a Chance that I caught HIV, bc I can not really sleep or eat. Also my period is not coming over all this stress...

Welcome to our forum and thanks for your question. Dr. Handsfield and I share the forum and share responding to questions. On this occasion I happen to pick up your question. FYI, Dr. Handsfield and I have worked together for more than 35 years and while our verbal styles may differ, our factual opinions and advice have never differed.

Your question suggest that you are still concerned that you may have acquired HIV or an STI from your encounter On March 2. Even before I saw your test results I felt that this was a rather low risk exposure. While you do not say anything about your partner, the fact is that most people, even people with risky lifestyles, do not have HIV or other STI’s. In addition, condoms are highly effective for prevention of acquisition of STI’s and HIV. Unprotected oral sex rarely leads to infection.

Sent your exposure in March, you report that you have been tested on multiple occasions. You should believe your test results. Tests for HIV are absolutely conclusive any time more than six weeks (42 days) after exposure.  Thus, your tests for HIV taken at 42 and 60 days both prove that you were not infected with HIV through the encounter you describe. Similarly your tests for gonorrhea, chlamydia, and Syphilis are also negative and should be believed.

I suspect that the reason that you continue to be concerned that you may have been infected from this encounter are due to the symptoms that you have experienced.  Muscle aches and the burning sensation that you have experienced in your chest are not typical symptoms of HIV, particularly this long after an exposure. With regard to your lymph nodes, if a person looks hard enough for them, most people will be able to detect mobile, small lymph nodes. This is normal. Even when lymph nodes are swollen (and your are not) they are non-specific and do not particularly suggest HIV, Particularly in a case where a person has a negative HIV test. If your swollen lymph nodes were due to HIV, your test would be positive.

I hope that the information that I have provided is reassuring and will allow you to stop worrying about the exposure you have described. Your tests have proven that you are not infected and the symptoms that you do describe are not suggestive of HIV or STI’s. EWH

My „sexpartner“ is afroamerican, he was a ProAthlete & came to Swiss with 28 y old, now he is 38.

My concerned is a lil, bc I did read that the population under AA is a lil higher.

Plus, when we were together he was very excited & he asked me if he can start penetrate me at start w/out condom. I told him No and he took a condom.

He told me he got tested in March, but who knows?!

Also something; he just had returned from his holidays from Ghana, what if he had unprot. Sex in Ghana & he got HIV 2? Would this pick the test?

Sorry for the long answer, I wanted to make sure to give you all the details & see about your assessment.

My muskle aches on my thighs sometimes go till to my knees.

Thanks so much for the additional information.  If you had not been tested already, this information might be important in evaluating your risk.  However, in your own situation your test results provide the most important information and your test results conclusively prove that you were not infected.  Please don’t worry.  EWH ---

Also with my small dicomfort node in my groin, can I stop worrying? 

I am sorry for the answers, it really concerns me & an a new situation with me.

A follow reteest at 10- or 12 week you would not recommend just for my peace of mind? Even when my groin node will persisst?

Also I had twice a hämatom on my knee once and other thigh. Is this the cause from anxiety/ muscle aches?

Yes, you can certainly stop worrying that the discomfort in your groin might be due to HIV or an STI. Believe your test results.

There really is no reason for any further testing for HIV. Your tests are conclusive and I am sure that they are not going to result. You do not have HIV!

Hematomas of the sort that you describe are not characteristic of HIV. While your knee discomfort may be related to your hematoma, the hematoma is not due to HIV.
