[Question #7054] Swollen Epididymis

Hello Drs - I recently came back from seeing my physician after finding a lump in my testicle. The lump was diagnosed by my general practitioner as a swollen epididymis (which was mentioned to be a result of a sperm blockage). The only recommendation given was an ultrasound test and the Dr forgoed an STI test given the fact that I've only had unprotected oral sex recently.
My exposures are unprotected sex w/ long term girlfriend until February 2020 then no unprotected intercourse with anybody and about ~10 unprotected oral sex exposures (I received) with 8 of those exposures being from March 2020 to June 2020 (all separate females). I've read that oral sex is a very low-risk activity for gonorrhea/chlamydia and deemed as "safe sex", however, given my swelling I was wondering if the two could be linked? I have had ZERO other symptoms (no urination pain or burning and no noticeable discharge. After googling the issue I am now having severe anxiety over this being tied to an STI as it would destroy my relationship which has been under tremendous stress as a result of me being a weak man under family & friends passing away. Please let me know your thoughts on my concerns and whether I likely am showing a symptom of an STI or if it's attributed to other factors. Could the swollen epididymis also just be a natural blockage? Have had consistent to better swelling for ~6 days now.

Two STDs, gonorrhea and chlamydia, are known to cause acute epididymitis. Almost certainly this is neither of these nor any other kind of infectious epididymitis. First, your symptoms aren't severe enough; infectious epididymitis causes more pain and more swelling than you describe. Second, you don't describe symptoms of urethritis (discharge, painful or uncomfortable urination), which usually are present when either of these cause epididymitis. You also don't describe symptoms to suggest non-STD urinary tract infection (painful, frequent, urgent urination), which usually are present when UTI is complicated by epididymitis. Finally, as you correctly state, you are at low risk for gonorrhea and chlamydia; oral sex indeed is low risk, especially for chlamydia.
For reassurance, it would be reasonable for you to be tested for gonorrhea and chlamydia, as well as urine culture to assure no other UTI. Discuss with your doctor. If and when done, I am confident the results will be negative. Either way, keep working with your doctor about the cause of this problem. I am confident it's not an STD of any kind.
