[Question #7134] New HSV2 diagnosis

I recently received a HSV2 positive diagnosis. Igg test value 1.12. I have not been sexually active for the past 14 weeks before testing. I had last tested neg for HSV2 in October. I’m a gay male and before that time I’d given and received blow jobs, rubbed penises together, and rubbed my penis on the outside of a guys butt. (I have been HSV1 positive for as long as I can remember with rare cold sores). I don’t remember any HSV2 outbreak. I recall that my penis skin maybe felt slightly sore for a day or so early winter. So: 1. how did I likely contract this? From receipt of oral or the the rubbing stuff? 2. Will I likely have future outbreaks? Will they be mild? Could I always be asymptotic? 3. And could I give it to someone else if they gave me oral? Or is it jjust from rubbing? 4. Could I be reinfected? 5. Do you recommend I take valtrex, or just if I get symptoms? Thank you!

There is probably a 95% chance that this is a false positive result. The CDC says anyone who tests positive for HSV 2 in the range of 1.1 to 3.5, needs a confirmatory test. The closer your value is to 1.1, the more likely it is that it is a false positive. You need a herpes western blot to confirm or deny this result. This test is done at the University of WAshington only. It is not available through traditional labs like Quest and LabCorp, but anyone can work with a provider and get it. Please - don't decide that you are infected at this point. You probably are NOT.

Thanks for this info. The test was HERPESELECT II AB.,IGG and it says reference range <0.90. I’ve had fairly frequent std testing. Previous results for this test were: 1.12 in July 2020; 0.81 in October 2019; 0.79 in May 2019; 0.43 in Nov 2017; and <0.2 in May 2016. They were all done by the same lab. 1. Does this upward trend over the years mean anything relevant to the recent test? I will definitely ask about the Western Blot. 2. Also, in terms of transmission can HSV2 be transmitted from giving or receiving blow jobs between men? Thank you again.

It means nothing whatsoever. The western blot is not well known among general providers, but you should investigate it on the internet. HSV 2 would very rarely be transmitted via oral sex. You can order the western blot it yourself to get things started. The UW number to call is 206-685-6066. It costs nothing to have them send you the kit required to draw the blood and ship it back but you will need a provider that will fill out and sign the lab requisition form. This is not something you can order yourself.
