[Question #7243] Std from being rimmed

Follow up from my last questions # 7129.


Hi doctor sorry to bother you again so aug 26 i got rimmed for like 5-10 sec max and they said it was no exposure or very little. 6 days later went to to the er to get tested for swab Of anus and urine of gc. The reason i went i have crazy anixety and i kept sweating from my but. Got my results back negative for both.

Kept sweating day 8 went to urgent care idk why i listened to the PA and he said just get treated and then the anixety would go away maybe it did.
I had calmed down a bit after i got treated but then yesterday i started having this weird feeling in my penis like a little pee stuck in the middle of my penis And had to pee alot!!when i squeeze my legs together a little pee drops out. NO BURNING WHEN I PEE THO.Been drinking alot of water cranberry and orange juice maybe its a uti?

I also been not eating well, smoking heavy weed cigs and taking a .25 xanax to help me sleep at night for2 weeks.

1.How strong is a gc swab of anus and urine test at 6 days?
2. Could have i given my wife an std got treated and got it back from her to my penis?
3. If i wanted to get tested again lets say how long should i wait now that i have gotten treated 8 days after exposure and today is 16 days since this happened so i got treated 8 days ago. (1000 mg zpack and shot)?
4. I have 1 pill of doxycycline should i take it?
5. What can you recommend me to about this little pee like stuck i think i got it before when i stressed out like this. Im scared to go to my uroligist and tells me i have a uti or w.e ill bug out.  
7. Is this a symptom of gc?
6. Can you give me some recommendations of what to do moving  forward? I plan on getting tested at 4 weeks. 12 days from now if i can hold up.
My wife not showing any symptoms already 16 days later thank god i have been running away a bit from her starting to think shes starting to relize something.

Also when dr hook told me i couldnt give it to my wife from my penis if i got it in my anus rea

Just went to the uroligist and my pee is good with  urinalysis no blood no sign of infection
Didnt want to tell him about a chance of stds cuz his a family friend

Welcome back, but I'm sorry you found it necessary. I reviewed your discussion with Dr. Hook, and it seems all your current questions already have been answered -- or the answers should be obvious from that discussion. Your symptoms almost surely are just the result of anxiety magnifying your awareness of trivial symptoms or normal body sensations that you would not notice except for your worries and anxiety about a sexual decision you regret. That exposure was extremely low risk for any STD; there are no STDs that are easily or frequently transmitted by oral to anal contact. (Oral STDs are uncommon, and when present generally are localized in the throat, and not saliva or tongue. So even if your partner had an oral STD, the chance of trnamission was very low.) And the contact was so brief that any minor infection risk was even lower.

1) I think you mean "how accurate", not "strong". The tests are excellent. A negative result is always conclusive.

2) Answered by Dr. Hook. Even if you had a rectal STD, you could not have transmitted it to your wife (unless maybe she rimmed you herself).

3) I see no point in being tested again. If you do so, the timing doesn't matter.

4) Do not take doxycycline. One pill will have absolutely no effect except maybe to make future testing inaccurate.

5,7) This sounds like pure anxiety, nothing else, and for sure not gonorrhea. However, I'm happy to see in the follow-up comment that you bit the bullet and saw the urologist. Of course I am not suprised your exam and urinalysis were normal. However, you should have told him of your concern about STDs. Standard testing without that knowledge doesn't usually include STD testing. A normal urinallysis is pretty good evidence against an STD, but not proof. On the other hand, there's no way you have an STD, so it really doesn't matter much. But in event of similar occurrences in the future, please give your doctor the full story. You can be confident that s/he will manage it objectively and confidentially, regardless of personal friendship.

6) What other advice? I hope these comments help you to finally move on. If not, consider professional counseling. It really isn't normal for you to continue to be so anxious and fearful after the repeated, reasoned, science based advice you have had, plus the negative tests.

Hi dr sorry i have more question sorry my ass starting getting wet again smh. I put toilet paper to keep me calm.

I went to the emergency room in the hospital again for a second test and they said they can only do the urine gc  i told them i already did the swab test here and pretty much the dr didnt want to do it.

1. Anyway the urine test came back negative does this have any effect if i had gc in my anus? 
( i know i was suppose to do the swab test and i think im just gona go to a gastro so i can relax my self finally).
2. Can a gastro do the swab test?
3.  I did do a treatment 8 days after i got a negative result from my 6 day swab just to be safe cause you guys told me i wouldnt be able to give it to my wife anyway and let the anixety go away and the wetness. It all went away But it came back today (my anus forsure more wet idk if its my hemorrhoids and im nervous but when i go wipe when its wet i get like a wet poop stain and like the toilet paper is wet a bit.   How effective is the 4 pills and the shot for treating someone? I take zpack when  I get bronchitis.

So now im scared that the 4 pills and shot didnt cure me or something and i got it back. I think so crazy but i know my body very well and that wetness by my anus wasnt there before this happened.

Sorry dr for being a nudge but this episode has put me over the top just trying to enjoy the holidays with my family and i cant.

Sorry you found a doctor with his head up his ass (instead a swab up yours) about STD diagnosis and management. (Feel free to quote me if you're in touch with him again!)

1) A negative urine GC test says absolutely nothing about potential rectal or throat infection. But since there's no chance you have gonorrhea, it doesn't much matter.

2) Any doctor can do a rectal swab test. With luck, your gastroenterologist will be more up to speed on this than the ER doc. However, be clear:  I doubt you have rectal gonorrhea. But as long as you're being tested, make sure it includes chlamydia. (In most labs, gonorrhea testing automatically includes chlamydia and vice versa. But it can't hurt to make sure.)

3) Oops -- I forgot you were treated. You don't say the exact drugs, but "4 pills and a shot" undoubtedly means ceftriaxone+azithromycin. There is no possibility of gonorrhea not responding, and very low chance of chlamydia treatment failure -- and chlamydia is almost never transmitted from an oral source, i.e. analingus is not a risk for chlamydia. So you can disregard my advice above:  it's pointless to be retested for gonorrhea or chlamydia. But seeing a GI doc is a good move to diagnose and advise about your anal moisture. Most likely nothing serious is going on, but better safe than sorry. (Maybe the GI doc will decide to test you for STDs anyway, just to help reassure you. Consider printout out this response to discuss with her or him.)

Ight dr had a ruff week. tried to get tested a few times this week...

Went to ny presbitory in queens twice again and they said they only want to do a gc test for urine. So stupid
Then i went to my pcp and he said i dont have anything and its all anixety i asked him if he can do the test he said he only knows gc urine test smh.
Also went to my gastro and he didnt have any swabs to do the test. Also he checked me and said it sweat. Again smh.

So i called a bunch of urgent cares finally got Lij urgent care that said they can do the test. Went and did it got my results back negative. It was a CTNGANAL test for GC.( when i got my results it said this is not a FDA approved test) i really hope thats the test!!!!!

This wetness hasnt gone away. 3 doctors looked at it and said its sweat and also confirmed when they checked its wet there.

Questions
1 did i do the right test?
2 should i test for anything else? Like stphillis?
3 im thinking of getting a botox shot in my but to stop the sweating Do you think i should wait and maybe i will stop sweating?
4 what would you do if you were sweating and it was causing you anixety ? What doctor Would you go see or just leave it for a bit and see if it goes away?

(I dont even believe Myself  i can even catch something from this exposure) but this wetness was not there before this happened and im trying to get rid of it. I feel like i developed something called swap ass!

Next step is going to the gym and sweating everything out!

The only reason im more scared is if i dont clean the sweat and w.e i might get a rash and then go crazy thinking syphills lol.

I just want to be dry down there! Fml

1) CTNGANAL probably was a nucleic acid amplification test (NAAT) for both gonorrhea and chlamydia. If so, it was the right test. Some NAATs are FDA approved only for genital testing, if the manufacturer hasn't submitted documentation of accuracy at other anatomic sites. However, all such tests are highly reliable for rectal testing. 

2) No, don't test for anything else.

3,4) These are not STD issues; we don't address non-STD health issues. These are things to discuss with your doctor(s). Personally, I would probably use talcum powder or cornstarch, wash the are a couple times a day, and then do my best to just forget the whole thing.

That completes the two follow-up exchanges included with each question and so ends this thread. This being your second on the same exposure and symptoms, it will have to be your last. Repeated questions on the same topic, especially when obviously anxiety-driven, generallyl are at risk deleted without reply (and without refund of the posting fee). ASHA is not keen on continuing to collect fees from anxious persons who will only get the same advice as before, and such questions have little educational value for other users, one of the goals of the forum.  If you find yourself still worried, with a "yes but" or "could I be the exception" sort of concern, I would advise re-reading both your forum threads:  almost certainly anything else that comes to mind has already been addressed.
