[Question #7755] Concerned HIV exposure

Hi Dr,
I had a body to body massage which I am not worried about in itself anymore and feel comfortable with that as i always see the same lady who is joint owner of the massage shop and I understand the risks are zero on HIV for massage and HE.
However, on this occasion the lady was on top and I just have this niggling feeling that my penis was pushed around the vagina and that the tip may have entered the opening of vagina during the interaction. I quickly leant down under with my hand and held my penis whilst she continued but I have this niggling worry about entering. I am circumcised and I don’t seem to have much feeling on the tip area and so makes me worry I didn’t feel it even though it entered her briefly just the tip of penis and not full length penetration.
When she turned round and lent over I could see that her vagina area was clean shaven and dry if that makes any difference.
I know a fair bit about her as we chat and she moved in with her boyfriend 6 months ago and is an avid runner doing 1 to 2 hour runs several times per week. She is super obsessed with her figure and staying fit and is 43 years old with a child. Not that this matters to any great degree but giving the full picture.

Welcome to our forum and thanks for your question. We receive many questions from clients who are concerned that their penis may have slightly entered a partner during masturbation or frottage. Obviously, such events are at least theoretically possible however in my experience they have never led to infection. Bus typically when a person enters a partner there is really no doubt that that has happened. Your partner sounds low risk but I realize that that does not change the concern you have expressed.
Other than what I have already said above, there is a little I can say to reassure you as to whether or not your penis entered your partner. Depending on your level of anxiety, testing may be appropriate in order to resolve your concerns. The fastest and most direct way to find out if you were exposed was to see if you can get your partner to have a test. If she is negative you are not at any risk whatsoever. On the other hand, if that is not possible, perhaps you should consider testing. At four weeks the results of a combination HIV antigen/antibody test would be more than 99% conclusive. At six weeks results of the test would be entirely conclusive. Personally I suspect there is no need for testing whatsoever but I also understand that sometimes the best way to address the anxiety is to test in order to be completely sure.

I have asked the lady owner questions and this is response speaking in third tense as on messages: “HIv? U come to massage And things u done in the room with the girls will not give u HIV. I do understand how I feel
But thinking positive that good thought

So based on including this feedback would you recommend testing (not considering my anxiety) for this incident? If only my circumcised tip entered for 1 to 5 seconds max then what would be chances of catching HIV.? Many thanks as always.

Thank you for the additional information. I think that this lady‘s reassurance is very helpful and it further assures me that, where are you, I would not pursue testing. As far as PEP is concerned, if you came to my clinic requesting PEP following this exposure I would not be willing to provide it as, in my judgment, the risk is too low to warrant this intervention. I hope this perspective is helpful to you. EWH ---

The chance that you acquired HIV from the exposures you describe is certainly less than 1 in 100,000 and probably substantially less than 1 in 1,000,000 that you were infected. (I.e. or, turning it around, chances are greater than 99.999 that you were not infected).
