[Question #7841] Unprotected oral sex - white discharge

Hello doctors,
I have been following your work for many years and I understand that unprotected oral encounters are generally low risk events from the perspective of STIs. I have a question about a recent experience.
2.5 days ago (Monday) I had unprotected oral sex with a woman. The following day I noticed a slight burning sensation when I urinated. In terms of 'pain' it was probably 2/10. The slight burning discomfort was concentrated near the tip. Over the past 2 days I've noticed a very slight white discharge coming out of my penis on 2 occasions. The last time was about 3 hours after ejaculating. I usually get a lot of clear discharge post-ejaculation (sometimes for an hour) so that's never a worry. But today, squeezing the top of my penis reveals a very small amount of white discharge. I usually don't get white semen discharge several hours after I ejaculate.
I don't have any other symptoms. The slight burning wouldn't be so much of a bother had it not been for this discharge I've seen (putting 2 and 2 together). During the oral encounter, the woman used an edible lubricant. She also kissed me and I don't have any symptoms in my mouth area. For what it's worth -- the woman seemed healthy and didn't appear to have any symptoms of STI at all.
Are my symptoms consistent with an STI (gonorrhea or NGU)? Is there any other info you can share? Testing at clinics is a bit complicated now because of covid lockdowns. I live in Ontario, Canada.

Welcome to the Forum. Acquisition of STIs following receipt of oral sex is relatively uncommon but when infection occurs the most common infections are Gonorrhea or non-chlamydial NGU caused by the introduction of mouth bacteria into the penile urethra.. typically symptoms do not occur until at least 3-5 days following exposure and sometimes longer. The severity of the symptoms can certainly be mild as well. Testing of a penile or urine specimen for signs of inflammation using a gram stain is the best way to determine if you are infected or not and to guide therapy, if necessary. I should also caution you that repeated self examination particularly with “squeezing” can cause precisely the symptoms you report.
Despite the challenges, I urge you to seek evaluation and to keep your hands off of your penis. If you are infected the signs and symptoms will get worse. If you seek evaluation, please do not urinate for at least an hour before your specimens are collected. On the basis of what you’ve said above, I doubt that you are infected and certainly would not treat you without proof of infection using the measures noted above.EWH

Dr. Hook,
Thank you so much for your response. I have been monitoring the situation for the past 4 days and here is what I noticed:
1. Throughout the day, I notice slight clear, sticky residue/discharge at the tip of my penis. I know what precum looks and feels like, and that it usually comes before and after sexual arousal/ejaculation. But this residue/discharge also appears many hours later and doesn't seem to be associated with sexual arousal.
2. During masturbation, I see slight white residue in my precum. It looks like semen, but that's very odd for me. I never see semen in precum unless I hold in my ejaculation and keep going.
3. I don't feel any pain anywhere (not when I urinate or anywhere on my penis). I also don't have any sudden urges to pee.
4. Does it matter that during the oral sex I had on Monday, the woman used edible lube and I didn't shower or urinate for about 6 hours after the ordeal?

There is problem with follow up questions. We permit up to two follow-up questions and comments for each clients questions. This response is your second reply. The additional information that you provide suggests that you may have NGU. In order to evaluate this I recommend that you do as I suggested in my original reply and seek evaluation from a professional who can evaluate a specimen for signs of inflammation and bacteria. Thus, as I said in my original reply, I recommend that you keep your hands off of your penis and seek professional evaluation. EWH
