[Question #7848] Oral Sex and STD

Hi Doctors.  I visited  an erotic massage therapist last week and she gave me an unprotected blow job.  The next day I had sex with my girlfriend.   About 36 hours later my girlfriend started having UTI symptoms (maybe STD?).  She has had quite a few UTIs in the past so dont know if this is just a bad timing coincidence.  She has taken Monurol (a UTI medication) which appears to have knocked out her symptoms.   I don't have symptoms other than feeling like I have to pee some.  I generally have a need to pee more anyways.  My urination urgency really started after her symptoms - psychosomatic?   I have no pain when urinating at all, swollen testicles, etc...   I did milk my penis this morning (6 days later)and saw a very small amount of discharge that was milk white.  I dont see discharge the rest of the day and have to really milk it in the morning to see any.  Is this much to be worried about?  Should I get tested?  Would the monurol work if it was an STD?

Welcome to the forum and thanks for your question. In general amongst sexual exposures, receipt of unprotected oral sex is relatively low risk. STI’s may occur following receipt of oral sex however the two most common STI’s which occur following oral sex are gonorrhea and non-chlamydial NGU which is caused by the introduction of mouth bacteria into the urethra of the person receiving oral sex.  Gonorrhea obviously requires treatment for both the infected person and any recent sex partners. There is debate however as to whether or not non-chlamydial NGU requires treatment since the bacteria causing the inflammation are normal mouth bacteria which have not been shown to cause complications or problems.  In your case, if the discharge you can express is truly white in color that is not normal and should be evaluated. The preferred test to evaluate this would be a Gram stain of a specimen taken with a swab from your urethra. Tests for gonorrhea and chlamydia could be done from the same specimen. The specimens provide the most accurate results when obtained when the person being tested has not urinated for more than an hour before the test is obtained.

Monurol (fosfomycin) Is not known to be effective treatment for either gonorrhea or NGU.

I urge you to seek a valuation and not to simply seek or accept treatment without testing if offered. Your Situation is complex. Urinary tract infections are related to sexual activity in women and your girlfriend’s UTI symptoms may have been just that. On the other hand if you have an STI you may want to be sure that she is treated for having been exposed the day after you received oral sex. The only way to be sure is to seek testing.  The Gram stain of the urethral specimen can give you useful results within a few minutes of having been obtained.

Thanks for the reply.  Fortunately, it appears the Monurol has cleared up all of my girlfriends symptoms.   I will be tested just to be safe.  As for discharge, I have only seen it once in the morning and it was not so much discharge as I had to "milk" my penis to see it; it was an incredibly small amount like a pin head.   I havent seen it any other time.   I am in another country and getting a gram stain appears to be quite difficult from my research.   A couple of quick questions.  I can get a Urine Gonorrhea and Chlamydia test.  Would that be accurate on day 7 if my symptoms were due to either?   If I had such a small amount of discharge, wouldnt it increase in velocity.   Lastly, wouldnt i have any other symptoms?  I do feel the need to urinate more often but my urine is fairly clear and i am thinking about it constantly.  Also milking my penis to see if I can find any additional discharge which I havent yet; just in the morning when I woke up once.

I did find a place that does the instant gram test.  I will take it tomorrow.  It will be day 7 post incident.  If that is negative, can I be assured everything is fine?   If it is negative, I bet the rest of my need to urinate goes away as i am so focused on it.

Thanks for your follow up. I’m delighted to hear that you have found a place to get a Gram stain and I would encourage you to do so. Please let us know what it shows. The comfort of having a test will be reassuring.

At the same time, a few more comments. These infections can be almost totally asymptomatic so the fact that you saw the discharge once Is still worth following up just for the reassurance that it has provided. Likewise, the fact that your partner’s UTI symptoms have resolved is also reassuring.  EWH

Thanks Dr. Hook.  I guess this will be my last reply.  The clinic did not do the gram stain as they couldnt see a discharge and thought the test would be highly unreliable; i didnt see any discharge this morning either when I woke up and milked my penis.  Instead, they had me submit a urine culture.   My significant other no longer has any pain, just some need to urinate more frequently.  She took the Monurol two days ago and it says it could take 3 days for all symptoms to subside.   Last questions below and thanks for the help.

1.  If my urine cultures are negative (at 6.5 days), does that mean I am in the clear?
2.  If my girlfriend had Gonorrhea or Chlamydia, would the Monurol get rid of the symptoms?  If not, maybe she just does have a UTI?
3.  Could we have gotten each other infected with NGU from our mouth bacteria?  Would the monurol work on NGU?  
4.  If my tests are negative and I continue to have symptoms, should I take Monurol for a potential NGU?

I did speak with the escort and she says she gets tested regularly.  Of course, that doesnt mean she gets tested in her throat which is the only way I could have caught anything.

Thanks for the follow up. I respectfully disagree with the clinic you saw and feel that a Gram stain might’ve been helpful. That said, this is done and said. Specific responses below:

1.  It is not clear to me whether your urine tests including test for gonorrhea and chlamydia or not. If the tests were performed and are negative, I certainly would not worry further..
2.  No, the Monurol would not be expected to resolve her signs and symptoms if they were due to gonorrhea or chlamydia.
3.  There are no scientific data on whether the Monurol would effectively treat NGU. Most experts do not believe that NGU caused by mouth bacteria need treatment or is transmissible.
4.  My advice is that if the tests are negative and your symptoms continue would be to seek treatment for NGU. This treatment would preferably be seven days of doxycycline taken twice a day. Alternatively a single 1 g dose of azithromycin would be affective as well.
