[Question #7850] Clear/yellow discharge. Previous NGU

Hi there. Just to preface , I’ve had a similar issue to this before about 7 months ago, I expect the answer to be similar but I’m just looking for a little clarification. The last time I had this discharge I was treated with a Zpack/flagyl/ and the shot for gonnoreah preemptively upon my request. This was after noticing minor clear discharge during the day, and it would be thicker and yellowish in the morning. Within 48-72h all my symptoms had resolved, and chlamydia/gonnoreah were negative when they called me with my results. My previous question explains that timeline in detail. So this time around I’ve been with the same girl that I had been having sex with in that question for the last several months. We broke up and I received unprotected oral from someone. I noticed that less than 48 hours later there was some clear staining in my underwear in the morning (didn’t think much of it) .. but I noticed this over the course of the next few days. Well today ( 8 days after exposure) I noticed the staining was yellow in the morning. It looked exactly identical to my symptoms from months ago. The clear discharge during day, and yellowish after waking up. I mean everything is exactly the same symptom wise. But ..the last time I had unprotected sex with the girl I had been dating was 2 weeks ago. (7-8 days before noticing clearing stains in underwear). I went to the doctor and they prescribed doxy(which I requested I instead of the zpack per your recommendation on my last question about this) and flagyl. I knew the flagyl probly isn’t necessary but took it anyway. I was not given the shot. I guess I’m just frustrated that now I don’t now if I got it from the new partner I received oral from or from the sex. It’s just strange to me I hadn’t had a recurrence for so many months ( and plenty of exposures to the girl I was dating) but as soon as I get oral from someone different these symptoms show up. Do you think this likely a coincidence? Also do you think I should’ve g

Gotten the shot for gonnoreah even though I tested negative for everything last time? I didn’t want to go overkill on the antibiotics. I should get the test results back in a few days but .. is it reasonable to expect chlamydia/trick/gonnoreah to be negative when I’ve experienced these exact symptoms before and it was negative? In fact there seems to be less discharge this time. Im just a bit dejected that I never found out what it was last time and I probably won’t this time either.. I know that can be seen as a good thing, I’m just a bit obsessive about this stuff and it sucks to not have a definitive answer. Whatever info you could provide I’ll appreciate! Sorry if it’s confusing be jumping between the last time this happened and what I’m dealing with now
This time:
Last unprotected sex with monogamous girlfriend was 3 weeks ago. I noticed first clear staining in underwear upon waking up 12-14 days after this and noticed it was yellow today .
Last unprotected oral with a different partner was 8 days ago. I noticed the clear staining in underwear 36 hours or so after this .. and noticed it was yellow today.. I know gonnoreah can present that quickly and can be transmitted orally but do you think this likely based on having identical symptoms 7 months ago and it all being negative ?
I was not treated with the shot for gonnoreah this time , but was seven months ago. Thanks again. Sorry for being so humbled!

Welcome back to the Forum. I'm sorry that this problem has recurred. I did review your earlier interaction with Dr. Handsfield and agree with all that he has said in that interaction. Based on your self-report, testing is appropriate - a cloudy or yellow penile discharge is not normal. You did not need the shot for gonorrhea at the time this occurred in the past. If you had gonorrhea, the test would have been positive - these tests are very, very reliable. It sounds as though you had non-chlamydial NGU last time. What is going on now will require testing to determine the best therapy. Following receipt of oral sex, the most likely diagnosis is non-chlamydial NGU but gonorrhea is a possibility as well. As Dr. Handsfield mentioned last time - " nonchlamydial NGU usually has no proved cause -- and happily, it usually isn't harmful to women".
Non-chlamydial NGU is thought to occur following receipt of oral sex due to the introduction of non-STI mouth bacteria into the urethra.

If the doxy clears my symptoms in the next 1-3 days can I safely assume it won’t recur? I just find it very odd that if this was from the oral and not the sex with my ex girlfriend that one exposure would lead to identical symptoms that I’ve had with someone else

Given that yellow urethral discharge is not normal, there are two major possibilities here- first that you have again acquired urethritis (NGU) from receipt of oral sex fro a new, casual partner. If this is the case, there are no good data as to why this occurs in some but not all or even most instances of receipt of oral sex. It does seem to occur more often following sex with a new partner as it was in your case. The second possibility is that the association with having a new partner is coincidental and something else is going on. Should it recur without having sex with a new partner, this possibility (something else is going on) should be considered through discussions and possible evaluation by a urologist who might look for other problems such as prostatitis. At the moment however, the association in each instance with sex with a new partner suggests that option 1 is the likeliest cause. EWH---

Okay thanks for the response. If the antibiotics clear it up can I assume I’m okay? It seems like it’s already improved since I started taking them 18 hours ago or so

Couldn’t it also be from the unprotected sex with my girlfriend? The symptoms just started so quickly after the oral. Is that a normal thing with NSU? Do we not have clear incubation periods on normal mouth bacteria? Please answer these two, this is what I’m most curious about

Yes, if your discharge clears up while on antibiotics I would not worry further.
While it is certainly possible that you have developed MG you from unprotected sex with your ongoing girlfriend, statistically it is more likely that the syndrome has occurred as the result of sex with a new partner. Of course, either is possible.
You are correct, the data are limited regarding the onset of NGU following oral sex. In my experience however symptoms can certainly occur as soon as 2 to 3 days after sex.
