[Question #7878] Hiv concern long term cough and groin rash

Hello Doctor,
I have had a exspsure many years back around 5 years ago where I woke up drunk inside a sex club in rotterdam, I had sex judging by the missing cash and no clothing worn, I really was in no fit state to make decisions like this. I am not sure on condom use or weather it broke as I don't remember removing one.
The sex worker was female and I believe possible Spanish or Romania of origin around 25 years of age
Since then I had a occasional groin dash that caused some red areas in the pubic area occasionally producing a spot or multiple spots, also I have had a groin rash with red line and similar spots in my groin fold, also having a sore broke skin area above my anus on occasion.
Around 3 months after this event I also had some red spots on top of my hands and occasionally had red spots on top of feet.
Fast forward 3 years and I have devoloped a cough that produces mucus from time to time mainly white and somethings tinged yellow, this cough has persisted although mild and more of a throat clearing cough for 2 years + I first noticed it after a mild cold, the cough has persisted with no other symptoms as such other than on two occasions I had chills and a fever very far apart, these only last a few hours and cleared with paracetamol, right now I am suffering the same groin rash also the cough has not gotten worse and I have not her symptoms apart from occasional shortness of breath but I put this down to anxiety
Can you please advise is this sounds like hiv? I am heading to my doctors this week for some testing

Welcome to our Forum and thanks for your questions. I can understand how you might wonder if a drunken misstep five years ago might be related to the symptoms you describe but this is statistically unlikely for the reasons I mention below. Testing will certainly clarify this. When you test, please be sure to be tested for HIV and syphilis using blood tests. If these tests are negative, I would not worry further about the possibility of an STI as the cause of the symptoms you describe.
To elaborate further, your risk for infection is quite low from any single episode, even if your partner had an STI, including HIV. Most commercial sex workers do not have HIV or other STI and most condomless unprotected sexual exposures do not result in infection. For instance, if your partner had untreated HIV, there is still a 99.9% chance that you would not become infected following a single sexual exposure.
Secondly the symptoms you describe are non-specific. The rashes you describe are non-specific and could be due to any number of causes. While syphilis can cause a variety of unusual rashes, again, what you describe is non-specific. Similarly, there are many possible causes of the sort of cough and respiratory secretions that you describe. As I said above, if your tests for HIV and syphilis are negative, I would not worry further that the events you describe from five years ago. I hope this information is helpful. EWH

Hi doctor I since had a result back from the doctors regarding a urine test that has high protein levels do you this look like hiv along with the other symptoms

The presence of protein in the urine is another non-specific finding and certainly does not suggest that you are infected. EWH---

Problem I have is multiple symptoms that all seem to mimic that of hiv of a long term ie more than 5 years, have you any idea what the hiv rates are like in Dutch sex workers? And would my risk be higher than somewhere else been what is considered a brothel, they call them sex clubs there. Also is there any know testing policy's at these places?

HIV rates among Dutch sex workers vary with a number of different factors. Licensed sex workers are, by law, checked regularly and rates are low. Rates are likely to be higher in unlicensed workers but still low, particularly if they also use IV drugs and share needles. There are not good specific data on rates in dutch sex workers however.
The problem with the symptoms you report is that they are all non-specific and searching the internet for specific symptoms all too often lists HIV as a cause of such symptoms. Your apparent anxiety serves to amply your detection of symptoms and the internet amplifies the relative likelihood that HIV is causing them. EWH

Hello, thanks for that info the club is definately a licensed venue as it serves alcohol and offers stag party's etc? Does this further strengthen the risk to be low? Club oq it was.

Also the rash in my groin has ulcerated and looks a lot like herpes which I am really concerned about

We provide up to three responses to each clients’s questions. Thus these will be my final response to this set of questions. FaceTime the comments above, I presume that the person you had sex with was an employee of the club. If this is the case, there’s certainly reduces the likelihood that that person had any STI., including HIV.
When would be in the usual place for a herpes outbreak and for a person to experience their first outbreak of genital herpes five years after exposure is likewise most unlikely. From the sounds of things however if you have open lesions in your groin I would suggest that you have them evaluated by a clinician. I am confident howeverthat they are not herpes.
