Dear Sir 

I would like to tell that 65 days back i had unprotected oral sex with sex worker. I am male. I saw one blister in her mouth. I can’t say the conditions of her mouth. After one month of exposure i took HIV ECLIA 4th generation test for all std. all came non reactive. Hiv also came non reactive . Should this result be considered conclusive. Though from the last 50 days i am having back pain and bit weakness. Please suggest . I am more concerned with Hiv .

Welcome to the forum. Thanks for your question.

You can relax:  for sure you do not have HIV. You had a zero risk infection in regard to HIV; the virus is not transmitted mouth to penis, or so rarely it can be ignored. (One estimate is that if the oral partner has HIV, the risk to the penile partner by oral sex would be once for every 20,000 exposures, which is equivalent to getting BJs by infected partners daily for 55 years before transmission might be likely.) Second the large majority of female sex workers do not have HIV, so your partenr probably wasn't infected. (And the condition of her mouth makes no difference in HIV risk.) Finally, the AgAb (4th generation) HIV blood tests are nearly 100% conclusive one month after exposure.

For a truly conclusive test, you could have another AgAb test, such as ECLIA, 6 weeks or more after the event. You can expect another negative result. Your symptoms don't sound at all like a new HIV infection, by the way.

So all is well. Personally, if I were in your situation I would not have been tested for HIV at all, and I would not have another test now. But that's up to you. For sure you don't have HIV.

I am thank ful to you for your kind reply. It helped. I have below concern which I would like to clarify -

1.Today is more than 65 days from the time of exposure. And i have went to orthopedic twice for back pain or rather pain in spine from neck up to lower back. He did blood test, x ray since then two times in each month. All came normal. Though in first report WBC count was 6.2 , after second month ( this whole second month i kept fast) it came around 4.2. Still in healthy range. At time i feel something like little itching in my mouth mostly at the inner cheeks but it goes away. This all happening to me after 15 days of exposure. And i went for test after 31 days of exposure with all these symptoms. Is this related to any STD 
2. Sir , i would like to know is it conclusive from report after one month of exposure that I don’t have any STD disease. Like hepatitis  b or c , after how many days of expansion i should go for teat to confirm that i don’t have std in my body. 
3. I too don’t want to go for HIV test as you have mentioned that mine exposure was of almost zero risk and hiv test after month was almost conclusive. But seeing the current symptoms like weakness ( especially when i get up in the morning, and at times back ache , bit uneasiness at times in my mouth) , hiv comes into mine mind. Tough to deal with. 

And i am also suffering from chronic sinusitis, allergic rhinitis and i have enlarged bone in my nose from a very long time.

1) Everyone's WBC count varies widely over the course of the day or week. The change from 6.2 to 4.2 has no health implications of any kind.

2) Like HIV, tests for hepatitis B and C are conclusive any time 6 weeks or more after the last possible exposure. But you don't need such tests:  you were at no more risk for these infections than for HIV, i.e. zero risk.

3) If your symptoms continue or you are otherwise concerned about them, you should see a doctor to learn the cause. It is 100% certain HIV is not the cause. And of course HIV has nothing to do with chronic sinusitis.

Dear Doctor 

I am grateful to you for your kind reply. Sir , below mentioned are mine concerns -
1. As informed to you that almost from the time of exposure i am experiencing pain at back (sometimes at lower back , sometimes between shoulders blades) and nose problems (nose blockage as i have grown bones, nose sinus, allergies rhinitis). With all these symptoms i did HIV test after one month of exposure as informed to you before and it came negative. 
2. But from the last few days i see bit growth in throat congestion and it goes away  ( it could be due to post nasal drip due to more block nose these days) and mild acid reflux. Very light. And i feel at times i have to breath deep or feel tired( again i think it could be because of nose blockage) . Actually Doctor (ENT) here has recommended for surgery to remove excess growth in nose. 
3. Seeing all this please , suggest if all this has to do with HIV as I don’t want to go for re test. Shall I go or not ? Does corona vaccine effects HIV result?

Symtoms mentioned doesn’t last long. They are their for a short duration and then goes away.

All human beings experience symptoms of the sort you describe. They do not sound like HIV at all. But even if they did, your test results prove 100% that HIV is not the cause. You do not need any more HIV testing unless and until you have new sexual exposures that might be risky.
