[Question #7940] Hsv

My partner and I are in a monogamous relationship and neither of us has had sexual contact with anyone else in at least 8 mos.  Unbeknownst to me, he occasionally gets cold sores on his mouth and has for many years. He has never had symptoms of genital herpes. A few weeks ago we had unprotected vaginal sex and he also performed oral sex on me.  He did not have any herpes symptoms but was experiencing a period of extreme stress.  I began showing symptoms of genital herpes 5 days later. I have tested positive for HSV-2 via swabs of my lesions (blood test was negative as this is definitely a new infection for me). My partner has not been tested. 


My question is related to the risk for my partner going forward. Can he now contract genital herpes by having unprotected vaginal sex with me?  I’ve read on the WHO site that he cannot bc he would already have the same strain of virus that he passed to me (in this case, HSV-2). But my OBGYN told me that his oral infection would not prevent him from getting genital herpes from me.  Is my doc misinformed? 


I understand that either of us could get infected in other parts of our bodies if we have contact with someone who has HSV-1, since we do not have that strain of virus, and maybe that’s why my doc answered the way he did.

I'm a little confused.  So he gets cold sores but you believe you acquired HSV 2 from him, is that correct?  So his cold sores aren't really relevant in this picture.  What is likely going on here is that he has HSV 2 genital infection and didn't know it and infected you.  Has he been antibody tested for HSV 2?  The person who has a history of cold sores is more likely to acquire HSV 2 without symptoms, which may be what is going on here. 
A person who has HSV 2, which appears to be both of you, will almost NEVER acquire HSV 1 subsequently.  Let me know if I am understanding this correctly

I thought either virus could exist in either area of the body?  He has never been tested.  I am making some assumptions  based on the fact he told me he has mouth sores but has never had an outbreak in the genital area.   But it sounds like you’re saying my HSV2 infection could not have come from his mouth sores, so yes he must have both and not know it.  He told me the cold sores go back to his 20’s, so it seems likely that he had that one first.  (We’re both in our 40’s now).

It sounds to me like he has both, yes, if you are certain that you didn't have HSV 2 before you had sex with him.  The IgG test misses 8% of HSV 2 infections compared to the western blot so the assumption that a negative antibody test means new infection is pretty accurate but not perfect.  Will he be tested?

I am certain I did not have it before. I have never had symptoms of HSV until now. And because I am single and sexually active I have my OBGYN test me for everything, every year.  I tested negative at one of those annual check ups just after I met him.  I usually practice “safer” sex (condoms), but stopped with him due to the length of our relationship. 

I do not think he will get tested as long as he and I are together.   He is regretful that I got infected, but seems resigned to the idea that it happened and no further harm can be done. 

Thank you for your help. It’s terrible how misunderstood this virus can be,  but I greatly appreciate having some solid answers.
