[Question #7971] unprotected oral sex and overnight stay with a female of unknown status

A brief background on me: I am a 36 year old male and I have penile lichen planus: it is well undercontrol and as you know it is benign and non-contagious. I have not had sex in the past 2 years since I really get freaked out after an intercourse.
After almost 2 years of absitinence, I went to a night club/restaurant and came across a woman. We interacted and got along. This happened in San Jose, CA (if the place matters). She was a divorcee and has kids and a dog. She seemed like a decent person of Fijian Indian decent.
While I was not intending on indulging in anything beyond just a few drinks and chat, but things kind of went on and we ended up spending a night at the hotel together.
Before I describe the incident, I recall that I applied topical corticosteroid to my inner foreskin (betamethasone dipropionate) in the am for treating the lichen planus rash and then completely forgotten about it. I also take the medication prozac (it is an SSRI for depression/anxiety).
So as far the incident goes, we booked a hotel room and we both stripped. I fingered her and he gave me unprotected oral sex. This happened multiple times over the course of time. There was a lot of hugging and kissing. Note that there was NO penetrative sex involved. I say this because I was very careful in not doing that since I know I would be freaked out after that I usually am very careful about the same.
We slept next to each other, woke up and made out again: fingering and she gave me unprotected oral sex. After that I cleaned my penis with water and we had a coffee and parted ways.
I asked her multiple times and she mentioned she is clean and free of STDs and was herself having a physical relationship after almost 2 years, well ofcourse I can not trust that verbatim.
Also, I reconfirmed with her that there was not sex between and she assured me that nothing happened, aprat from her giving me unprotected oral sex and me fingering her multiple times over the course of the night ofcop

Hence am very concerned and would like to understand what are my risks of any STDs: hiv/hpv/anything at all. I am looking for a long term partner and would like to make sure am clean before indulging with my long term partner

Also note that prior to this incident, I have done all the tests for STDs and am negative for all of them. This is the only incident of concern since in the past almost 2 years I have not had a physical relationship.

Welcome to our forum and thanks for your confidence in our services. I'll be glad to comment. The exposures you describe were low risk for acquisition of STI's, including HIV. HIV is not spread through kissing, touching, our mutual masturbation despite the fact that it is typical for partners to get each others' genital secretions on each other during the process.
Further, please remember, most people do not have STIs. She assured you that she did not have STIs and most people do tell the truth when asked.
The only part of the exposure you describe associated with any STI was your receipt of oral sex. As a biological process, oral sex is an inefficient means of transmitting STIs. Most STIs are not transmitted at all in this way. For instance, there are no proven cases of HIV which have been acquired from receipt of oral sex, oral HPV is rare and when present is typically not transmitted through oral sex, and chlamydial infections are almost never transmitted through receipt of oral sex. The major SITs acquired through receipt of oral sex are gonorrhea a or non-chlamydial NGU. The majority of persons who acquired either of these infections become symptomatic within 4-7 days of exposure.
You don't mention how long it has been since your exposure. If you want to be absolutely sure you were not infected, a single urine test for gonorrhea and chlamydia is the main test I would recommend. Such a test would provide accurate information any time more than 4-5 days after your exposures.

The expore happened on Friday, June 11th night .. Saturday, June 12th morning. I have not had any symptoms per say, atleast not as of yet. The woman was 48 years old, if that matters.

I wanted to be a 100 percent sure since, I am not clear if usage of topicals to treat penile rashes like psoriasis/eczema/lichen planus, etc make the area more susceptible to STIs/STDs?

Your symptoms have occurred far too soon to be due to HIV. further, your partner's age places her in a low risk category for HIV and other STIs.
Your topical steroid will not influence your risk for infection. EWH

Dr. Hook I actually mentioned I have had no symptoms at all for gonorrhea a or non-chlamydial NGU testing you suggest doing :)
I will nonetheless do these tests. Just to reconfirm, one last time, I do NOT need any other STD/STI testing before resuming normal relations with my long term partner, is that correct?

Thank you. I am aware that you have no symptoms of gonorrhea or other STI‘s and suggested testing not because I believe that you are likely to have an infection but for the reassurance that it will provide you. My sense is that you are quite anxious over this misstep and would benefit from the testing I suggested as reassurance. Personally I would not test but as I said before, I suggested testing for you for reassurance purposes only. I am confident your tests will be negative.
I hope this final response is helpful. I see no reason for additional testing beyond What have I have suggested and again suggested that only for the reassurance that I sensed you sought. If you do not feel the need for testing certainly you do not need to pursue it. EWH
