[Question #8010] HIV test and condom question

Good midday Doctors:

1) I had unprotected vaginal sex w a woman of unknown HIV or STD status back in April 11th. I started and completed PEP. At exactly 42 days after finishing PEP I had an HIV ab/ag 4th generation lab blood test and was negative.  
Is this test conclusive? 

2) I have had pain on my upper left jaw and went to my dentist she said my gums were inflamed. I haven’t had a teeth cleaning in a year and have been lax on brushing lately and grind my teeth at night. So she thinks a good teeth cleaning will help. So I googled “inflamed gums” and a cause could be HIV/AIDS. Could that be a sign of acute  HIV infection? I’m r is that for late stage infections? The google site has no context.

3) I had protected sex and oral sexual w a friend but 2 hours earlier I poked the tip of my tongue w a dental plastic flosser. Somehow I did that. It was like a pin poke and my tongue bled for like 10 min. Two hours later I performed cunnilingus on her. If I had HIV would she be able to get it through cunnilingus from the tongue bled? It wasn’t bleeding at the time that I know of and it was 2 hours later.

4) I put the condom on but I started to put it on inside out. Then I reversed it and put it on the right way.
 So the side of the condom that touched the head of my penis for a quick second would be on the outside. Then that side, the side that was on the head of my penis for a quick second,  was on the outside and then went in her vagina. If I had Trich would she be able to get it that way?  

5) my face has been red today could that be an HIV rash at this point? Or is the test I mentioned in question 1 conclusive?  

Should I worry? Syphilis, gonorhea and clamydia tests were done at 6 weeks from the April 11th encounter and were negative. I got the second vaccine shot for Hep B and have antibodies to show the vaccine took hold.   Thanks for your detailed answers in advance.

This is your sixth set of questions related to low risk exposures over the past six months, including four or five on the exposure for what you took PEP unnecessarily. This sort of anxiety driven questioning is discouraged and future posts about low risk exposures may be Closed without a response or return of your posting fee.  The answers to several of the questions you posed below have been answered previously and those answers will not change.  Straight to your questions:

1.  You have asked this question repeatedly and the answer remains the same. Your results are conclusive. No further testing related to the April 11 exposure that you are perseverating about is necessary.

2.  You have been repeatedly warned about the large amount of misinformation present on the Internet which is discovered through Google searches. This is an example of that. You have a negative HIV test and gum disease it has many, many causes. This is not a manifestation of HIV.

3.  This is a silly question. You do not have HIV. Even if you did, cunnilingus would not put your partner at risk for HIV.

4.  When a condom is put on, removed, and then put on again with the reverse side now exposed, if the person putting the condom on has an STI they could potentially/theoretically  expose their partner to it affection. The risk of infection however is very low.

Thanks for the info! I do have some questions about gonorhea and clamydia transmission.  

1) if I have protected vaginal sex with woman A and her vaginal fluids get on areas of my body not covered by the condom (like base of penis) and then have protected vaginal sex with woman B a couple days later….can gonorhea and clamydia transmit that way? 

2) syphilis can be skin to skin transmitted I’ve read, is that only when a sore is present?

I should clarify:

can woman B get clamydia and gonorhea from me based on woman A (assuming she has it) having vaginal fluid on my skin?

Your anxiety-driven, unfounded questions serve no purpose.  Once again, please understand that HIV and other common STI’s are transmitted only through unprotected, penetrative sexual encounters With infected persons. Most persons, including most commercial sex workers , are not infected. Further, most exposures to infected partners do not result in infection. With that background I will briefly address your anxiety end driven follow-up questions.

1.  This question has been answered repeatedly. Cutaneous exposure to infected general secretions is a no risk event.  In the unlikely circumstance that your partner had any STI, the fact that that person’s genital secretions might get on your non-mucosal skin in no way represents a risk for infection.  These sorts of “what if” questions are entirely without any usefulness.

2.  Syphilis transmission occurs only in the presence of direct contact with an infectious lesion. In the absence of skin lesions skin to skin contact is a no risk event.

I did have a protected sexual encounter on June 26th. I got tested for everything including syphilis on July 23rd. 27 days later. 

1) is testing for syphilis 27 days later conclusive?

2) I did have a pimple like thing develop on my arm 10 days after and it’s till healing 3 weeks later. If vaginal fluids touched that part of my body could one get a syphilis cancre there? 


3) can one have a cancre and still test negative at 27 days?

4) Lastly, if one gets someone else’s blood on their finger, who’s HIV status is unknown, and they have a cuticle or top layer of skin missing from a healing cut, would that be a risk factor for HIV? Nothing on their hand was actively bleeding.

As you know, this will be the final responses. Also, I must point out that these questions are repetitive and the answers were readily obtained by reviewing past responses to your earlier question. Onto the responses:

1. Most experts will tell you that a interval of at least 30 days it’s necessary for results of Syphilis blood tests to be conclusive. Despite that, it is far, far, far more likely than not that your test will remain negative and you did not require Syphilis from your exposure of concern.
