Hope you are doing well. This is follow up of question no 7897.
As informed before , on 18 March i had unprotected oral sex with sex worker. I am male. She did blow job. On 18th April i did ECLIA test 4th generation for all STD. It came negative. Though from 10 April i started having bit body ache which is bit more these days. During our last conversation you said no need for further test. I didnt went. I was at ease until following happened-
1. Bloating , constipation and nausea from the last 5 days.
2. Body ache mostly at the back and it is not continous. I feel stiffness in my neck at times.this has being from a long time.
3 i feel swollen lymp nodes under my jaw on rigtt side. Though its not paining that much.i felt it today but i am not sure from how long it was their as it under my beared. So far i did not had any fever or sore thorat.
In your opinion do i need to go for hiv test again as the last one was testes at 31 days afyer exposure. Secondly , i was corona vaccinated long time back , does this could have affected hiv test result ?
I am very much concerned with lymp node that hae formed. Is this could be due to any other STD apart from HIV.Thank you for your support and cooperation.

Welcome to our forum and thanks for your question although I am sorry that you felt the need. As you may know, Dr. Handsfield and I share the forum and on this occasion I happen to pick up your current question. In preparing to reply, I reviewed your interaction with Dr. Handsfield and agree with all that he said. In particular I would point out that he has already told you that the exposure you described was an absolutely no risk event. There are no proven instances in which a person has acquired HIV as a result of receipt of oral sex. That, coupled with the fact that it is unlikely that your partner had HIV on a statistical basis, combine to make this event no risk event. No testing was necessary to begin with and there is so certainly no reason for further testing. While I certainly concern understand your concern regarding your risk for having acquired an STI from the exposure you described, your testing as well as multiple studies prove there is nothing to worry about at this time. In response to your specific questions:
1. Bloating, constipation, and nausea or non-specific problems and are not associated with recently acquired HIV. On the other hand such problems may be a reflection of anxiety.
2. These are non-specific complaints. They certainly are described as being part of recently acquired HIV however the timing of your exposure was so long ago that you should be looking for another cars. Your Intermittent back aches are due to something other than HIV. Anxiety could be causing symptoms of the sort.
3. These too are non-specific problems. Everybody has lymph notes which, if they look carefully enough, can be found. The isolated lymph nodes you describe are not due to HIV and, any tenderness your experience is likely to relate to your poking in efforts to look for lymph nodes.
In summary, I see no medical or scientific reasons direct him and additional testing for you. The complaints that you describe are not suggestive of HIV infection. Further, I would add, there are no data to suggest that the Covid vaccine in any way interferes with the accuracy of HIV testing results. Believe your results. Please don’t worry. EWH

Thank you for your kind response. I was not worried util i had this lymp node which i have noticed yesterday.
1. Its being more than 3 months since i had that exposure. That was my first sex with some one from outside. I mean i was ver loyal to my wife until that day when it happened. I regret to what happened. I want to know if lymp node swelling is ARS due to HIV isn’t its happening too long after the exposure? What is the time duration for ARS to happen? Now when i have noticed its almost 103 days from the time of exposure?
2. I underwent ECLIA 4th generation test at 31 days after exposure. It was blood and urine test for complete STD’s. All came negative. How much conclusive my result is after 31 days of exposure?
And please let me know can I continue unprotected sex with my wife?

Dear Doctor
You have mentioned in point number 2 “ They certainly are described as being part of recently acquired HIV ….” . This sentence of yours is not clear to me. And as informed when i went for hiv test i was already having symptoms like body ache but not that much.
Regarding lymp node it’s on my face close to ear . I feel pain when i press it. Its on the right side and bit swollen when compared to left side of the face.

Your follow-up questions are a bit repetitive and are indicative of your unwarranted anxiety over your no risk exposure. Straight to your follow-up questions:
1. As I told you, your exposure was a no risk event. The ARS occurs within a month (typically at 2-3 weeks) following exposure. It is too late for your current symptoms to be due the ARS and your symptoms are too mild. Persons with the ARS are quite sick with simultaneous high fever, severe muscle and joint aches, and a severe sore throat. As I also told you above- "Everybody has lymph notes which, if they look carefully enough, can be found." The lymph node swelling of early HIV is widespread and not isolated. Your submandibular lymph node swelling is NOT due to HIV.
2. HIB tests of the sort you had are more than 99% conclusive at 4 weeks (28 days) and entirely conclusive at 6 weeks and beyond. Your other STI tests performed at 31 days are conclusive.
There is nothing about the events you describe that indicate any need to avoid unprotected sex with your wife.
I hope you can accept these answers and move forward. You did NOT get HIV or other STI from the exposure you describe. EWH
