[Question #8052] #8002 follow up - Is this HPV?

Hi Drs. 
My question is an update / follow up on #8002. I was sexually active before marriage with CSWs occasionally. My wife has this enlarged lump on her lower left neck. I'm convinced with the Dr's reply that I did not pass on HIV. 
Our primary physician prescribed a pap smear test as a regular screening process. My wife is from the eastern part of the world. She never had a pap smear before nor a HPV vaccine. I am her 1st sexual partner.  My concern here is , did i pass any HPV to her, given that she never had a sexual partner nor was never vaccinated for HPV? Will the pap smear come out positive on HPV? We have been in a physical relationship starting 8 months ago. Please help

Welcome back to the forum.

From your previous questions, it seems you have been sexually active with at least a few partners, including female commercial sex workers. You can assume you have been exposed and perhaps infected with HPV. 90% of sexually active people acquire at least one HPV infection. You have mostly (entirely?) used condoms for vaginal sex, but condoms are only partially protective against HPV, so you could well have been infected. In the event your wife's pap smear shows evidence of HPV infection, it's possible it was not from you, since on rare occasions female virgens have HPV -- that is, rare cases may be nonsexually acquired. However, if indeed she has never had other sex partners, then most likely you would be the source, and you should be prepared to explain the circumstances. (My understanding is that many women in your situation are well aware that their husbands had other sex partners before marriage, so perhaps this would not be a problem.)

Most likely her pap will be negative. But you need to be prepared for that possibility.

Hi Dr. Handsfield. Yes, I was active and there were a couple of instances I didn't use condoms.  Nonetheless, as you pointed out I probably have had HPV. My wife never had any sexual partners before me. My concern here is, did I have active HPV in me and I passed it to her ? Why do you think the pap test will be negative probably? And if I did pass on the HPV, would it cause any symptoms?

You are re-asking the same question in different words:  "did I have active HPV in me and I passed it to her?" is what I answered above. It is possible. However, you could have had HPV that is now gone and not transmissible; and even if you have active infection, it isn't always transmitted during sex. As I said above, more likely than not your wife's pap smear will be negative. If she ever does have HIV, most likely it will cause her no symptoms or serious health problems. Most HPV never cause symptoms or serious health issues. But there are no guarantees. I suggest you and your wife discuss the situation and consider whether she should be vaccinated against HPV, which will protect her against 90% of all possible health problems from HPV.---
