Good afternoon Dr. 10 days ago on a trip I had an encounter with a sex worker, there was no penetrative sex, only masturbation, but she gave me a prostate massage with a device covered with a condom that she unwrapped in front of me. The device had not been previously used by her, but my concern is that she put the condom on with the same hand that she had been touching her vagina a minute before, and spread lubricant on it with the same hand. then he introduced it into my rectum for prostate massage.
Is there any risk of HIV assuming that the hand with which you put the condom in the device had some traces of vaginal fluid? Do I need to take a test? Can I have unprotected sex with my wife?

No risk and no worries; you can safely continue unprotected sex with your wife. HIV is sexually transmitted only by insertive sex -- i.e. penis must penetrate a partner's vagina, rectum or (rarely) mouth. The virus is not transmitted by hand-genital contact.
However, I do not completely understand your question. The title mentions "prostate massage", but the question says nothing about it. You also do not state what "device" was used. Probably these details make no difference, but I would be happy to comment further if you would like to clarify the situation.

The “device” was a dildo with several small balls in a line. I don’t know if you understand, (my English is not as good as I would like)
It was covered with a new condom, but my fear is that some vaginal fluids could contaminate the external surface of the condom before introducing the “dildo” into my anus. She put the condom and the lubricant on it with the same hand she fingered her vagina some minutes before

If I correctly understand the epidemiology of HIV in Spain, the chance any particular sex worker is infected is very low, under 1% and probably under 1 chance in 1000. And when a male has unprotected vaginal sex with an HIV infected women (for 10-15 minutes), the average risk of infection is around once in every 2,500 (2.500, i.e. two thousand five hundred) events. So what could it possibly be with the small amount of vaginal fluid that might be on the outside of a dildo? For these reasons, there has never been a reported case of HIV transmission in this manner.
All things considered, your risk of HIV was very, very low. I do not recommend testing, and if I were in your situation, I would continue unprotected sex with my wife without any worry.

Forgot to say that 4 days after the described encounter I got diarrhea that still last until today.
Could it be related to hiv from the described encounter?

Diarrhea is not likely to be the only symptom of a new HIV infection, and 4 days is much too soon: symptoms cannot start sooner than 7-8 days after infection.

Try not to overthink these issues. There is no significant risk of HIV or other STI.
