[Question #8081] HSV-2

My boyfriend had 10 panel STD test which included blood igg test for HSV 1 and 2. It came back as 2.66 high reading for HSV 2 which is positive. I am extremely concerned as we did NOT have vaginal or oral sex he did have his underwear off but I did have mine ON and we were simulating sex but I never removed my underwear. What is my chance of getting HSV2 from him this way??? I assume he is assymptimatic as he has never had any outbreaks. Do I need to get a rapid blood test?

The IgG test has real problems in the low positive range (1.1 to 3.5) with accuracy. There is about a 50-50 chance that his result is a false positive. He most certainly needs a confirmatory test, the herpes western blot is the best from the University of Washington. He can work with his own provider to get this test or with me at westoverheights.com. He should NOT simply accept that he is infected.
And the contact he had with you presents zero risk for transmission of HSV IF he is truly infected. You do not need testing related to this encounter, no.
I hope you and he will take my advice and get the confirmatory test. The CDC now strongly recommends this due to the poor accuracy of positives in the low range. In fact, just yesterday, I had someone with a positive at 12 and he did not confirm with the gold standard western blot. we desperately need better herpes antibody tests that are easily available.

Ok I was under the impression that if his penis somehow made contact with my vagina as maybe the underwear shifting when he rubbed against it I could be infected?? I Iwill definitely have him get retested since he does not have a provider can he go to any lab and have his blood drawn or does it have to be a special way he said he went to the website you gave me but he is not sure how to have the blood drawn. If that test comes back not detected can I consider this 100% accurate I don’t want to put myself at unnecessary risk.

Yes, he can order this test himself. And there are several websites - STDtest.com is one, healthcheckusa.com is another.
If the test is negative for HSV 2, there is a 92% chance, compared to the gold standard western blot, that he is truly negative. I still don't believe the contact you had is concerning. If you've had sex before in your lifetime, did you also do the full panel of testing?

Yes I have done the full panel but not the western blot. So you are saying the western blot would be 92% effective

No, I'm saying that the IgG test picks up 92 cases for every 100 that the western blot picks up,so it is 92% sensitive for HSV 2.
