[Question #8091] Follow up

Hello Dr. i apologize for returning. As you know 60 days or so ago i asked a question. I did test of Day 8 after have a brief exposure that i didnt ask for (oral). All my results were negative. For Gon/Chlamydia. However based on your recommendation i had sex with my wife. Despite being negative for all 8 days after exposure i did have some weird tingling which i thought may be NGU. My PCP gave me doxycycline. I took it for 10 days.  Then had sex with my wife again. 2 months later i still feel tingling and some burn when i pee. Not much, but it lingers. Could there have been a possibility i had NGU gave to my wife, took antibiotics, them she reinfected me? Possible? I took a AZO UTI strip test and it showed positive for Leukocytes but not nitrates? Is there possibility the 8 day test wasnt conclusive? Would love your input? Should i test for STD and talk to my wife at this point.  I feel so violated from all of this and dont know if its all in my head now.

By the way. I took 3 Azo strip tests today all negative for nitrates and some slight purple reading indicating presence of leukocytes.

Welcome back to our forum. I’m sorry that you felt the need. My assessment is unchanged following our interactions two months ago. Further, You have proven that you were not infected with direct tests for gonorrhea and chlamydia as well as used less sensitive tests (the Azo tests) on multiple occasions which also showed no evidence whatsoever of infection. Finally, despite the absence of both risk AND signs of infection, you talked to your own physician into giving you a course of doxycycline which is the recommended therapy for chlamydial infections and NGU. You did not acquire an STI from the exposure of concern.

As both Dr. Handsfield and I mentioned to you two months ago, when persons begin to scrutinize themselves for abnormal sensations on urination or in the genital areas, they often become aware of normal sensations which typically go unnoticed when persons are not looking for them. These normal sensations can also be heightened or aggravated through repeated self examination or general manipulation. I suspect this is what is going on.  My sense is that your guilt and personal disappointment in yourself over the encounter you described has led you to be hyper aware of normal sensations. You need to figure out how to move forward. Counseling may help this but further treatment or diagnostic tests will not.

Actually the AZO shows positive for a UTI.

Thanks for the clarification. These nonspecific azo tests still have limited accuracy, particularly when weakly positive and has already stated above, there is really no reason for you to think that you had an STI of any sort. EWH---

Ok. Thanks. What your saying is there is 0 probablity of acquiring a STI from my exposure. If that's true, ill own that.

Then why a detection of leukocytes in UTI test?

As you know the form provides three responses to each clients question. This is my fourth and final response. As I mentioned above, the Azo test is non-specific, subject to contamination on collection, and may have false positive results. There remains no evidence that you acquired an STI for me no risk and counter you described. It is past time for you to move forward. Address your anxiety and guilt. I say this out of concern for you and nothing more. EWH---
