[Question #8110] ITS

Hello again dear doctors.I hope you are fine. I promise this will be my last questions because I know that the site policy does not allow repetitive questions, but please answer me for the last time. These are not anxiety driven questions, I just want to be 100% convinced.
After the possible exposure in June when I had multiple condom protected sex, both orally and vaginally with a sex worker ( in the same evening),I did the following tests:
- 6 days post exposure: NAAT- Gonorrhea and chlamydia- negative
-41 days post exposure :  Anti - HIV 1+2(ser ECLIA- HIV combi)- negative; AgHbs ( ser ECLIA) - negative; RPR syphilis(ser)-  negative; Antibodies Treponema pallidum - negative;
-43 days post exposure: Anti - HIV 1+2(ser ECLIA- HIV combi)- negative; AgHbs ( ser ECLIA) - negative
-48 days post exposure: Anti - HIV 1+2(ser ECLIA- HIV combi)- negative; AgHbs ( ser ECLIA) - negative
Can I 100% rule out these infections? ( HIV, Hepatitis B and Syphilis)
From the previous posts I know that statistically by this age, 42 -   I had / have HPV  but what is the possibility of HSV 2 infection after this exposure?
I read different posts on your site but I can't  understand statistically speaking ( in percentages) in what is the possibility of infection after such exposure? So far (2 months post exposure) I have not noticed any genital lesions, just a recurrence of oral herpes that I have since childhood. If these lesions appear, how long after exposure/infection can they appear?
Thanks for your  understanding !!

You do not need to promise not to return with further repetitive, anxiety-driven posts as I can assure you that any further questions of this sort may be deleted without any comment and without return of your posting fee.  the questions you have listed above have already been answered.  Answers will be brief:

1.  Your tests are conclusive.   As you have been told before, you do not have HIV, syphilis or hepatitis B acquired from the exposure you described.  Your tests prove this

2.  As a sexually active person, it is likely you have acquired HPV at some time in the past.  This warrants no intervention or therapy unless you develop lesions.  There is no evidence you acquired HSV from the exposures you have described.  There are no scientifically derived statistics to quote regarding the probability of infection however, as you have been told before, the likelihood approaches zero given your condom use and the nature of your exposures.  You are far beyond the period when HSV lesions would be expected to be seen had you acquired the infection from the encounter you describe.  Lesions typically appear within 8-10 days after exposure.

Once again, these repetitive, anxiety-drive questions must cease.  The answers are not going to change. EWH

Hello dr. Hook!
I am sincerely sorry that I,ve created the inconvenience for you to keep repeating the same questions over and over again. Since I have two more follow-up questions (and not to lose it) I want to ask you if you will be kind to answer me the following ( informative and educational only)Ș
1. The fact that I took only 13 days of doxycycline and not 14 days as is the standard treatment for syphilis could somehow mask the test results ,not to prevent / cure the infection if it had occurred and give a  false negative result? Is it relevant that the test was done at 41 days and not at 6 +weeks
2.Can you tell me what are the causes of prostatitis in men? Can stress and anxiety trigger this condition? From what I discussed with the urologist, prostatitis is recurrent. can you tell me what would be the indicated treatment in this condition?
3.A person who already has HSV1 is less likely to become infected with HSV2 ,than a healthy person? ( in case of exposure to the virus)
4.What are the STIs that can be transmitted through French kissing and under what conditions?
5.In what situations can hepatitis B, be sexually transmitted? Is the hepatitis B virus found in tears, saliva or vaginal secretions?
Thank you in advance and have a nice day!

1.  Thirteen days of doxycycline would be curative if syphilis in the unlikely circumstance that you had it.  your test results are accurate.  Believe them!

2.  STIs do not cause prostatitis.  Other bacteria from the urinary tract occasionally cause prostatitis and in many patients there is no obvious cause.  If you have additional questions about prostatitis, please see a urologist.

3.  Yes, infection with HSV-1 reduces the risk for acquisition of HSV-2.

4.  Hepatitis B is transmitted by penetrative ano-genital sexual activity.  not by oral sex, not in tears, and not due to exposure to saliva from an infected person.

Thank you for the answers.
1.Can you tell me if Igg  antibodies remain detectable for a long time after a treatment (successful/unsuccessful) for syphilis?Or what type of antibodies remain detectable and for what period?
2.In a previous answer to my questions  you mention that HSV 2 infection is not real in my circumstances? What would be the circumstances for contacting this infection?
3.What are the STIs that can be transmitted through French kissing and under what conditions? If I come in contact with an oral sexually transmitted infection, what are the symptoms?
4.I suspect that hepatitis B is transmitted only through unprotected sexual contact? when you said that it is not transmitted through oral sex, you were referring to cunilingus or fellatio?
Thank you and I hope you will clarify me with answers.

Final answers to these repetitive  question.  Please remember my warning about further posts:

1.   IgG antibodies are often present following treatment for syphilis.  The antibodies may be present lifelong.
2.  Your exposures were condom protected.  You have not demonstrated lesions suggestive of infection, Further HSV-2 is virtually never acquired from receipt of oral sex (I realize you also had vaginal exposures but these were condom protected as already mentioned.)
3. None other than HSV or syphilis transmission due to lesion contact (lesions are typically visible or at least detected)
4.  Sigh, hepatitis B is not acquired through oral sex- receipt of oral sex, or performance of oral sex on a partner
