Merhaba, çalışmalarınızı takip ediyorum. Başıma gelen bir olayı sormak istedim. Kötü ingilizcem için özür dilerim. Bir eskortla cinsel ilişkim oldu. Tarih 29 Ağustos. Sadece korunmasız genital bölge ve apozisyon. Bunun ne kadar sürdüğünü bilmiyorum. Bir öpücük gerçekleşti. Eşimde HIV olabileceğinden şüpheleniyorum. Belki. Hello, I follow your work.  I wanted to ask about an incident that happened to me.  I'm sorry for my bad english.  I had a sexual relationship with an escort.  The date is August 29.  Only unprotected genitalia and apposition.  I don't know how long this took.  A kiss took place.  I suspect my partner may have HIV.  Maybe.  I went to 3 different doctors.  Dr.  1, “There is no risk of contamination.  However, if there is acne, lesions or irritation, there is a possibility of transmission.  2. dr: There is absolutely no possibility of contamination.  3. dr: If the genital frottage and apposition period is long, transmission may occur.  Now I want to ask you with confidence.  Does time matter?  Absolutely no penetration.  If I have small lesions on my penis, if vaginal fluid comes into contact, is it possible to transmit HIV?  After 3 days of contact, I checked my penis hard and the pain started.  No discharge, no burning in urine.  An uncomfortable feeling towards the tip and foreskin only.  When I got up to the toilet in the morning, there was no pain, then it came back.  I went to the doctor and the dr said there may be small lesions but this does not increase the risk of HIV transmission.  Is this pain psychological or did it occur after a strict control?  I have a wife, I feel guilty.  Is testing necessary for such a contact?  Should I have unprotected sex with my partner?  Even if this genital frottage, rubbing and insertion is prolonged, is it possible to transmit HIV by contact of vaginal fluid with my penis and urethra?  Please let me know, doctor, I trust you.  There is no burning in the urine. Is testing necessary for such contact?  Should I have unprotected sex with my partner?  Even if this genital frottage, rubbing and insertion is prolonged, is it possible to transmit HIV by contact of vaginal fluid with my penis and urethra?  Please let me know, doctor, I trust you.  There is no burning in the urine.  An uncomfortable feeling towards the tip and foreskin only.  When I got up to the toilet in the morning, there was no pain, then it came back.  I went to the doctor and the dr said there may be small lesions, but this does not increase the risk of HIV transmission.  Is this pain psychological or did it occur after a strict control?  I have a wife, I feel guilty. I went to the doctor and the dr said there may be small lesions but this does not increase the risk of HIV transmission.  Is this pain psychological or did it occur after a strict control?  I have a wife, I feel guilty. I went to the doctor and the dr said there may be small lesions, but this does not increase the risk of HIV transmission.  Is this pain psychological or did it occur after a strict control?  I have a wife, I feel guilty. Is it possible to transmit HIV by contact of vaginal fluid with my penis and urethra?  Please let me know, doctor, I trust you.  Is it possible to transmit HIV by contact of vaginal fluid with my penis and urethra?  Please let me know, doctor, I trust you.

Welcome to the forum and thanks for your confidence in our services.

There's a lot of information there, but it seems clear in translation.  The most important information stops with the first few sentences. Genital apposition without penetration carries no risk of HIV, and kissing is not a risk. Most female sex workers and massage workers do not have HIV, but even if your partner were infected, you were not at risk and therefore your wife also is not at risk. I agree with the advice you heard from your doctors:  no risk. (Except I disagree with Dr. 1:  there is no known increased risk by exposure of acne etc to HIV infected fluids. Any such increased risk is very minor and can be ignored. Finally, it takes exposure to large amounts of virus for HIV to be transmitted:  the amount from vaginal fluid contact with your penis or urethra is not enough. This is why genital apposition is no risk.

From a strictly medical standpoint, you do not need testing for HIV. However, you might feel better if you are tested:  the negative result probably would be more reassuring than anything I can say. In the meantime, it is safe to continue to have sex with your wife; you will not be putting her at risk.

I hope these comments are helpful. Let me know if anything isn't clear.

Dear Dr. Thank you for your quick response.  Could you answer for Doctor 3?  Confused about time.  Dr 3: What he says can be a risk if these contacts are long-term.  Do you agree with this?  If genital apposition, rubbing or frottage takes a long time and vaginal fluid contacts the penis and urethra for a long time, is there no risk again?  Is there a risk of exposure to intense virus from vaginal fluid in prolonged contact?  I don't know how long the genital apposition lasted, but it wasn't short, it was a little long.  Finally, what can cause penile pain?  Another infection?  Or maybe that's why it's like holding and pulling while controlling it hard?  Or psychological?  Because it wasn't there when I woke up and then it came back?  Just pain, an uncomfortable feeling and a feeling of wetness.  But there is no wetness.  No burning. Thank you very much for your service, Dr.

I wanted to add.  Do small lesions on my penis carry a risk?

No skin lesions of any kind, on your penis or anywhere else, make any difference in risk.

Dr. 3: There are no data. Common sense says a more prolonged exposure might be more risky than a brief exposure. However, NO HIV transmissions have ever been known to occur by genital apposition, so I doubt duration of such contact makes any difference.

No STD causes penile pain, but that's as far as I can go. We address STD issues, not all genital symptoms. However, whenever someone suspects his or her own symptoms have a psychological origin, usually s/he is correct.

Don't over think this event. Probably nobody has ever been infected with HIV from an exposure of this kind. You will not be the first!

Hello again, Dr.  I noticed a new symptom.  I don't know if it was before or after sexual contact.  There are wart-like spots near the head of the penis.  As far as I've read from here, an hpv doesn't show symptoms this early.  I searched the internet and it looks like penile papules disease.  My question is this.  If penile papules are present before sexual contact.  Or if it's a wart.  If vaginal fluid contact occurs during genital apposition, frottage.  Does it pose a risk to hiv or other std?  This is how I read information on the Internet.  Should I be worried?  Do you still think there is no risk?  In this situation.  Need testing for hiv or other std?

Correct, visible abnormalities due to HPV do not appear sooner than a few weeks after infection. Pearly penile papules probably are more likely, but they probably are not new. More likely your anxieties are making you you are notice something that has always been there.

I still believe you are at no risk for any STD and see no need for HIV/STD testing -- except maybe for reassurance:  re-read paragraph 3 of my initial reply.

That completes the 2 follow-up exchanges included with each question and so ends this thread. I hope the discussion has been helpful. Do your best to stop worrying!
