Hello dear doctor, we talked before, but I forgot my membership and I just opened it.  My question is about sexual contact there.  On the day of sexual contact, I was sure it was just genital apposition.  But I think a lot due to stress and I doubt whether there is a 1-2 second vaginal contact.  I was sure that day only apposition.  But now my memory is not clear.  My penile pain, which occurred 3 days after sexual contact, continues, now it is the 9th day.  And 1 day ago my left neck started to hurt.  I'm just worried.  Could I have touched the inside of the vagina during apposition?  If there was, would I definitely remember?  What is the risk for HIV in such a situation?  I'm planning to test for HIV RNA PCR and HIV DUO on the 14th day.  If it's negative, is there a chance it will come back later?  Have you encountered such a situation?  Is penis pain just stress?  Any std possibility?  There is no burning in the urine.  Thanks doctor.

This thought is stressing me out.  When the penis is rubbing between the vagina.  The penis was rubbing between the inner and outer lips of the vagina.  Any chance of contact inside?  Would I remember?  I have std 10 test results and they are all negative.  Are the 5th day std 10 test results reliable?

Welcome back to our Forum.  As you might know, Dr. Handsfield and I share the Forum and today I happened to pick up your most recent questions and will be resounding.  In preparing my response I reviewed your recent thread and agree with all that Dr. Handsfield said.  Genital apposition/ frottage is a no risk event.  The presence of acne lesions makes no difference and while it is logical to think that prolonged exposure MIGHT increase the risk, this is unproven and there are no know cases in which frottage has been proven to lead to infection.  The risk of the encounter you describe is effectively zero.  You are more likely to be struck by lightning today than to acquire HIV from the exposure you describe.

If penetration had occurred, you would have known it.  I suspect that what is going on is that your guilt and anxiety have caused you to doubt your memory- please don’t..    we have a term for what you are going through- genitally- focused anxiety.  This causes persons to doubt the accuracy of their memory and to notice and focus upon normal sensations which would otherwise be overlooked.  In the absence of any new lesions, your penile pain does not suggest any STI.

A HIV DUO assay combined with a PCR test at day 14 would detect virtually all recent infections although most experts would still conservatively then follow that test with a repeat DUO test at 4-6 weeks.

Doctor, as you say, I'm worried. This question may be silly, I'm sorry. No penetration I'm absolutely sure. But. When the penis is rubbing between a vagina. What would be the risk if the head of the penis came into contact with the vagina for a short time? I have such a concern but I am not sure.

Would I know for sure if there was such a thing? If only there was a little contact. would i remember this? Is this just a concern?

He also started having rectal pain. I have pain in the rectum and penis. There are bruises on the praeputium part of the penis. Like purple veins or spots. Could these be any HIV symptoms? Or any std symptoms? Can pain in the rectum and penis be prostatitis?

Your multiple, somewhat repetitive, replies and follow up questions are indicative of your unjustified anxiety. Your five and 10 day test results prove that you did not require a typical mucosal STI such as Gonorrhea or Chlamydia. Obviously, those results are too early to provide definitive information on HIV or Syphilis, neither of which you are at risk for.

Your concerns about possible penetration are groundless and again, is already addressed above, simply show your anxiety. You need to think clearly about this and move forward. The exposure was a no risk of that. EWH

Dear doctor, I want to put an end to my anxiety. But today is the 10th day. And under the tip of the penis. Where the foreskin is. I noticed a cancre spot. Small and white like skin. Not swollent. Just looks like a crater and has a white spot in it. I googled the photos and couldn't find anything similar. But I am not sure. I have a few questions.

1-) Syphilis or chancre cause pain in the penis?
2-) Can chancre come out in 10 days?
3-) My contact was low risk but I want to do an HIV RNA test on day 14, is that certain?
4-) Can a 14-day HIV RNA negative test be positive later on? Have you seen such a situation?
5-) Along with penile pain, there was pain in the upper part of the anus last night.  Like the feeling of pooping.  What can cause both penile and rectal pain?  Is this a std ?
6-) You said a 5 day std panel test for gonorrhea, chlamydia for sure.  So what can cause this penile and rectal pain?
7-) If I have any STD from my partner, will my risk of contracting HIV increase?  Same sexual contact.

I try to control my anxiety and not think. I wake up with a new symptom. Now a place that looks like chancre and again I'm worried. Escort girl from central asia region Tajikistan. That's why I'm worried. HIV and other std. Because when I asked he said there is no std. But. I said I would pay for the test and he didn't want to come. That's why I was afraid. I think he may have lied. Because he no longer responds to my messages. Lastly, if my penis came into contact with the vagina for a short time during genital frottage. If the partner is HIV-infected. What would my chances of being infected be then?

Occasionally, there is a tickling sensation in the penis and rectal area.  It's like being tickled or aroused. It looks a feeling of pressure in the rectal area.

Final responses as part of this thread.  Following closure of the thread, I must warn you that further anxiety-driven exposures related to your close to no risk exposure may be answered very briefly or not at all and that any posting fees will not be returned.

Straight to your follow-ups:
1.  Syphilis chancres are typically painless and take, on average three weeks to appear.  To acquire syphilis through the exposure you describe would be most unusual.  OTOH, the trauma or repeated self examination can cause pain and discomfort.  Also there are penile glands and so-called pearly penile papules which may be what you are seeing. If you are concerned, you should see an experienced clinician who can examine you
2.  See above.  This is not impossible but earlier than average.
3.  Repetitive.  As I said above "A HIV DUO assay combined with a PCR test at day 14 would detect virtually all recent infections although most experts would still conservatively then follow that test with a repeat DUO test at 4-6 weeks."
4.  See above. I have never seen or personally heard of a person who developed a positive test after a negative PCR at 14 days.
5.  This is not an STI. There was no anal/rectal exposure.
6.  I have nothing to add here.  Please review earlier responses.
7.  The presence of STIs increases susceptibility to acquisition of HIV.  Nothing you reported suggests that you are at risk for HIV.
