[Question #8299] Reactive Arthritis/Synovitis Std?

Hello Doctors,I hope you are both well, after 4 months I’m back with some questions, I have some questions over my incident 9 months ago.I hope as I’ve given a long period and no place else to turn due to Covid and most doctors so busy and worried about the weird systemic illness I have, I have no idea if it chronic pelvic pain syndrome or chronic Prostatis or something else.In my last post I mentioned I did a MPX HIV1/2-HBV-HCV PCR at freedom health and everything came back clear that was at 5 months post my incident, I also thought i could put it behind me but did another std test at 6 months negative all,I was happy to put this episode behind me.But my symptoms still continued and I had another appointment with the urologist, this time he did a prostate massage and I was found to have enterococcus and Staphylococcus infection, nothing had appeared on my urine tests previously for months.I was put on amoxicillin for a month to try and clear it.My friend who is a doctor said those kind of bacteria are expected to be found there and was kinda confused on the antibiotics given to treat this.I was also given both shots of the Covid phizer vaccine, but a few weeks later I did an antibody test and it came up I have no antibodies for Covid.This was quite alarming and made me worry I wasn’t making antibodies.I also went on to get Covid in august confirmed by Pcr.I got an extremely mild infection and really wouldn’t have know unless I was tested.Since Prostatitis I have had joint shoulder and neck pain and had a scan, it’s show I have synovitis and maybe reactive arthritis, I’m very scared that I have an std that is not showing due to everything that has happened.Dr Hook on med help says reactive arthritis can be due to hiv and chlmydia I’ve tested so many times it crazy, for all std’s apart from MG, herpes, Trich or HPV.My last Hiv and std test was at 9 months post incident, 19 hiv tests in total.
I have severe back pain, gastrointestinal inflammation and shoulder pain.

I'm sorry, but your use of the forum to repeatedly ask similar anxiety driven questions, when all evidence has shown you have neither HIV nor any STI, is excessive and must end.

I'm sorry you got COVID, but happy to hear it was mild and no longer an issue for you. That you did not later have detectable coronavirus antibody is not an indication of poor antibody production for other conditions. Among other things, there are several kinds of COVID antibody tests, and some are expected to remain negative after infection. In any case, the HIV tests you had are not dependent on antibody for positive results (because you have had RNA and antigen tests, in addition to HIV antibody). It remains 100% certain you do not have HIV.
It is conceivable you indeed have chronic bacterial prostatitis, not CPPS. It is true that the bacteria found on culture can be normal in the genitourinary tract, but they also can cause prostatitis. You should follow up on this with your own doctors. They are also in the best position to determine if you might have reactive arthritis (which may in some cases be triggered by prostatitis). You are in superb hands if you are continuing care with Freedom Health, a highly regarded and highly respected clinic for all issues and conditions related to sexual health, including ReA. Neither Dr. Hook nor I have any expertise they do not, and they probably have provided personal, hands on care for more patients than we have. They will know how to evaluate you for ReA, perhaps including HLA genetic typing. (They'll know what that is and how to go about it.)
This forum is not a site for evaluation of problems other than HIV or STIs, which unequivocally you do not have. As you have been told before, we do not permit repeated question on the same topic, so this must be your last. Thank you Dr Handsfield for your incredibly fast response, I understand what the science says and have been given antidepressants and therapy to overcome my anxiety’s.
I’ve stopped the antidepressants and desperate to get on with my life.
Unfortunately I’m not in the hands of freedom health as Dr Cummings has left and now they are called the wright practice headed by dr wright, they are a private and extremely expensive medical organization that I can’t afford to go to more than once.
I’m on the waiting list with the NHS to see a specialist but that’s not until March, Covid has destroyed the NHS waiting times and patient appointments, so I hope you can be generous with me and see how difficult it is to get answers.
1.) Does reactive arthritis, synovitis linger even if an infection has passed?
2.)Would amoxicillin clear up any enterrocus or staphylococcus infection?
3.)the amount of antibiotics I’ve taken in the past, Doxcy, timethorpin, Ciproflaxcin and now amoxicillin would have cleared any bacterial Std if there was one.
4.)penile Mordors disease/ thrombosis that I experienced 3 months ago it has cleared, is it suggestive of an infection?
Thank you once again, honestly I’m a normal sane English man, I just want to get on with my life,I understand no risk exposure and definitel tests results but these last years of Covid have made it impossible to get answers and with my symptoms it’s made me go doo lally!

Sorry you're having trouble getting further evaluation on account of COVID. I don't have any insights about getting around such restrictions in the UK. But this forum does not provide direct medical care and is not a substitute for it.
2) Enterococcus, almost certainly yes. Staph -- depends on the staph species, of which there are several; and testing to determine antibiotic susceptibility. To be discussed with the doctor or clinic that ordered the test.
3) Yes. Now stop asking about STDs.
4) I have not heard of and am entirely naive that that disorder. Looking online, I find Mondor's disease (not Mordor's) as a rare, superficial thrombophlebitis that can occur anywhere on the body, including the penis. To my knowledge, it is not triggered by infections or STDs, but I can't say anything more about it. Not being an STD means it is out of this forum's intentions, which do not include genital disorders other than STDs.

Thank You Dr Handsfield, I know you get fed up with a lot of these repetitive questions, I hope you understand how amazing it is for us to talk with somebody with as much experience and knowledge along with Dr Hook.
To speak with Dr wright at freedom health just on the phone costs like a weeks wages, it’s crazy how much money they are making whilst the NHS fails to see people.
To finish up with and then I promise to leave you both in peace, I hope in knowledge that you helped me, even when you were blunt and abrupt I know it was out of kindness.
I was suicidal at some points and was convinced I had hiv and was ready to take my own life, it’s still difficult to move on when you have had such a low.
My physio wants to give me a cortisone for my shoulder I think I will go for it as it will help me carry on until I have time to see the specialist.
To finish off with my list of symptoms from the start.
1.Urinary tract infection/frequent Urination
2.Chronic Prostatis/CPPS
3.swollen lymph nodes
4.LPR Reflux
5.diverticulitis/Colitus
6.Oral Thrush
7.Penile Mondors disease
8.Reactive Arthritis
What’s next?
So I think you would agree a proper mad list of symptom’s to drive anyone to depression and despair with crazy anxiety too.
In your opinion is this some kind of inflammatory condition caused by a bacterial infection or injury
Literally started the evening/next day after the hand job and lasted to this day nearly 11 months later.
With all my tests I’ve done 19 over a 9 months period, I would literally be in the Guinness book of records and history books right if in turned out to be a std.
Dr wright at freedom health said they would have to rewrite all the text books on Hiv if some how I got it and bet me a million pounds that I don’t, said it was impossible.

As I already made clear, I have no suggestions and will not speculate about your diagnoses or underlying conditions, except to reiterate that you have neither HIV nor any other STI. You are not in the running for that particular Guinness award and I agree with Dr. Wright.
That ends this thread. I hope the multiple discussions have been helpful at some level, but it seems clear that you are still having trouble accepting or believing the repeated, reasoned, science based reassurances you have had repeatedly both on this forum and from the various doctors and clinics you have seen. I imagine I or Dr. Hook have suggested professional counseling previously. It still makes sense.
