[Question #8306] Follow up oral sex risk

Hey doctors, me again!
I really hope this dosn’t fall into repeat anxiety based questions it is about the same exposures but they are new questions.
Thanks for the help before it really fixed my anxiety about it all and happy to report that life at home is as good as it ever was.
I am starting to think about it again and just have a few follow up questions for complete closure.
So I’m worried about Chlymidia
Recap I had a few msm receptive oral exposure outside marriage over the course of 2020. Obviously I regret every one of them.
I ended up going to docs for some none specific symptoms that I was prescribed doxy for.
Day 5 of these I did a STI test as I realise that I could have been at risk. Test was negative
Questions
1) with me taking doxy and 5/6 days of this took the test if I had a long standing infection would this have still been positive? I have read a study that showed it takes 5-6 days after Zpak to go negative is this the same for doxy or with it been a longer course is that 5/6 days even longer if so this proves that I never had infection for any time in the year and I can relax. Bashh say it takes up to 3 weeks to turn back negative is this true
2) how long does it take doxy to completely clear Clym (I guess this is same way as asking 1)
3) the reason I’m obsessing about the tests is I’m worried that I caught something in say June 2020 passed it to my wife between then and Jan (when I took the doxy) which cured me and my wife is still living with Chlym. I have tried to rule this out by having extra tests at a month and three month after treatment (while having sex with her) I have remained negative - is this enough time to catch it back if she had it or will I remain immune for a period of time?
4) are we still staying with all new info that chlamydia is negligible risk for oral sex in msm? Or is it seemingly more important of a risk now we know it lives in the mouth?
I guess I’m worried because while I remain symptomless. Me and my wife are struggling to c

Struggling to conceive, something I read Clym Is notorious for.
She also has started having irregular periods as in late or early.
I have tried to get her go to the docs for this but she just shrugged it off as stress.
Is irregular late or early period a sign of Clym

Welcome back to the Forum. On this occasion I happened to pick up your questions and will be replying. I’ve reviewed your earlier interaction with Dr. Handsfield and agree with all that he said. I think you are overthinking this and worrying unnecessarily. As Dr. Handsfield said, the efficiency for acquisition of STIS from giving or receiving oral sex is lower than with genital or rectal exposures. Chlamydia in particular does not “like “ the oral cavity. Further, when present, oral infections may clear over time without therapy. Finally, in the unlikely circumstance that you had chlamydia, your doxycycline would almost certainly have cured it. I really think you are overthinking this. In reply to your specific questions:
1. 5-6 days of doxycycline Would cure most chlamydial infections. The issue of persistent tests following therapy is complex. While most infections have been cured and are no longer detectable following 5 to 6 days of therapy, in a small proportion of persons, DNA from the bacteria may still be present and detectable. While your negative tests may indicate that you have infection and were cured, statistically it is far, far more likely that you never had the infection at all. Determining which is truly the case is impossible
2. See above. Even after the infection is cured and can no longer be transmitted to others, in a very small proportion of cases they chlamydial DNA may still be detectable.
3. Believe your test results. If your wife had chlamydia and you Had been infected in the past and cured, you could become reinfected. Your repeat tests prove that you are not infected.
4. Correct, the risk for acquiring Chlamydia through performing oral sex on an infected person is negligible.
As I mentioned above, I really believe you are over thinking this. There are many causes for difficulty in conceiving. Do not let your guilt over your missteps cause you to overthink this. You have no need for further treatment and no need for further testing. You do not have chlamydia. EWH

Thank you Doctor! I promise (and I know I don’t need to as you will just delete any repeat questions along this line without refund anyway) that I won’t pay or post for any further questions in relation to this even.
If I could just ask Statisically or scientifically if my wife had Clymidia how likely would you say it would be for me to have caught it back within three months. I know this is an unlikely event but would be interesting to know. We have sex regular obviously otherwise that would be the reason for not conceving haha.
So here is my thought process even though I have been stupid over 2020 it’s statistically unlikely that I got Clymidia from oral sex. Oral sex is safer reviving than giving (bashh words not mine), Clym dosnt like the oral cavity so could assum my partners had no oral clymidia infection but even if they did they could have cleared it before meeting me and even if they still had it it’s unlikely for me to catch it via them performing the act on me. Further more I I can forget about my latest exposure as there was no sex between that and finishing doxy so I only have two exposures recently that could have even theoretically caught it. There is a slim chance that even if I was cured a NAAT test at day 5/6 could still have been positive. And finally a test at 3 months is a pretty good sign that both me and my wife are Clymidia free.
I think if we put numbers to this we are talking numbers in the millions/billions that my wife is carrying Clymidia.
I know I’m harping on about Clymidia because I have ruled out Ghonnaria and the others.
I’m going to use the rest of my words to say a massive thank you to yourself DR handsfield and everyone else who runs this site. Honestly if you read the internet you would assume it’s almost a given to get an STI every time you shake someone hand. It’s great to get some expert advice.

Results of what is this question serve no purpose however, if your wife had chlamydia and you had had sex more than three or four times you would have most likely require the infection. Please stop your needless worrying. EWH ---

Thanks doctor that’s very reassuring to know.
Can I just ask finally. I have noticed two other symptoms, and I know I’m in the deep end with this as no way would someone without anxiety notice this but it’s very real.
I have occasional cloudy urine - so you can’t see through it. I noticed this first time when I did the test in April but also a few other times when peeing in a cup. It’s not just dark it’s certainly cloudy with a whitish colour. This only happens occasionally other time it’s normal. I saw a post about adding viniger which I have does and it seems to clear it somewhat.
Second thing I noticed with my urine is occasionally there is a strand in it - looks like a white worm. Again this is not always present but have noticed it a few times. This was never present in the many urine tests I done so worried this is new and could change your decision.
I take it from my tests I can rule out Ghon and Clym (as well as HIV and Syph tested after 12 week)
The problem is my local GUM and docs won’t do any more tests they are sure I’m 100% clear so I would have to go private if I needed to test for anything else. Which i really can’t afford.
I have tested 3 x urine dip test first void
1 x full urinalysis at lab
3 x Clym and Ghon
2 x Syph and Hiv
What about mycoplasma gen? Is it at all a risk cheapest test I can find is £100 and I cannot afford this. The GuM won’t test without discharge. I’m at a loss with this it seems it can harm my wife if I have it so of course I need to rule it out.
Please tell me what I have to test next to 1000% rule out any complications to my wife. She is all I care about now.

As you know, this will be my final replies as part of this thread. In addition I must warn you that any further anxiety driven questions to us related to the encounters described above may be deleted without a response. You continue to seriously overthink this. Final responses:
1. Cloudy urine is a notoriously unreliable indicator of the presence of infection or STI. Similarly seeing what appear to be “threads” in your urine is likewise unreliable and may represent normal genital secretions. The fact that they are Internet likewise is strong evidence that they are not due to an infection.
2. Mycoplasma genitalium is not a concern. Doxycycline, which you have already received is somewhat active against this organism. Furthermore and more importantly, mycoplasma genitalium very, very rarely if ever is required to do a receipt of oral sex.
This completes this thread. It is will be on the time for you to put your fears aside and move forward without concern. You’ve proven that you did not require an STI from the exposures you have described. Take care. EWH
