[Question #8309] Assess a need for HIV testing.

Good day,
I am a male from Poland.I was tested 3 times for HIV with negative results (my last post here 3,5 years ago - condom breakage with CSW). Now I am in a new relationship and afraid to pass anything to my new girlfriend. There were 6 encounters with 6 different women who receive money for sex (all Polish) - however these were not typical escorts - rather students looking for additional money. I consider them higher risk since almost all of them swallowed my semen - however unlikely to be drug addicts . 3 or 4 of them claimed there were tested for HIV sometime in their lives, and I saw negative results from one of them. All encounters involved protected vaginal sex ( I think two intercourses per one encounter on average with each ) and unprotected oral sex performed on me ( avg. 2 times on each encounter).The condoms never broke as I check it everytime.  Aside from these encounters I had about 10 unprotected oral sex on me without intercourse by another two CSW's.  I have recurrent genital warts . About 6 protected intercourses were with genital warts on uncovered part of penis - that time I didn't know it were warts, later confirmed by a dermatologist. 2 intercourses were with an abrasion form rough oral sex on me before sex on the uncovered part of my penis. A few other unprotected oral sex on me was quite rough resulting maybe 2 times with small nicks and once little bleeding from few sites on penis, after warts removal (it reopened), a Aside from these, there were : deep kissing, once with cancker sore and inflammation in my mouth , one time rimming on me and once my bleeding finger in one's girl mouth after cutting my self while opening a bottle - didn't notice it before I put it in her mouth. I have tested on all STI's(negative) aside Syphilis and HIV.  Please, assess my risk for HIV considering genital warts (some info on internet state that warts let HIV enter when in contact with vaginal fluid) and other factors mentioned and need for testing. Welcome back to the forum. Thanks for your continued confidence in our services.

The quick answer:  of course you need to be tested for HIV! That's the only way to know whether or not you are infected. Clearly you have been careful, and having had sex only with women makes your risk quite low. However, it is pointless to attempt to judge the risk of HIV or syphilis based on the details of the various events you describe. Statistically, it is very unlikely you have either one, but testing is the only way to know for sure.

HPV and genital warts have little effect, if any, on risk of HIV if sexually exposed. (Studies on this have given conflicting results, but the weight of evidence is that there is little or no increased risk.)

So I recommend you be tested for both HIV and syphilis, and there's nothing more I can say until you get tested. I'll be happy to comment further if you would like to let me know the results. In the meantime, I hope this information is helpful. Let me know if anything isn't clear.

Doctor, thank you for your answer. 
I always tried to play safe which is always condom protected vaginal sex without breakage, avoiding anal sex. However the relatively high number of risky partners and some circumstances I did not have always influence on like cuts, warts etc. made to seek testing in a week (6 weeks since my last episode ). For know there is only a wait for definitive results. Once again thank you for taking your time. Take care

Good day,  I tested negative today for HIV, Syphilis and HCV - all with finger prick tests. HIV rapid test was BioSure Self Test. However the timing was 7 weeks past my last exposure (condom protected sex with abrasion at a base and genital warts + deep kissing with inflammation and being biten on my lip during kissing. Should I still re-test ?

I'm glad to hear of your negative test results. I'm not very familiar with the Biosure test -- not much used in the US. But I understand it's a highly respected AgAb (4th generation) test, and all AgAb tests are considered conclusive any time 6 weeks or more (actually, 45 days) after exposure, So I see no need for additional testing for HIV. Your syphilis and HCV results also are negative-- although HCV was a waste of money and effort:  HCV is never sexually transmitted between men and women, or so rarely that it can be ignored. One calculation from Scandinavian investigators (Denmark?) reported a transmission risk from one episode of unprotected vaginal sex, if one partner is known to be infected, at one chance in 190,000.
