[Question #8312] Receipt of oral sex

Dear Experts
Male here. I received oral sex in Vietnam from a massage parlour worker. Because of this and the price I believe she was not one of the high end prostitutes. However, before this I was not aware of what HPV was and after finding out about it, I have become extremely regretful and anxious that I contracted it.
More details of the encounter: I did not get an erection but I still put a condom over my penis and since it wasn't erect, I am unsure how much of the penis was even covered or if it was loose. She gave me oral and also masturbated me but I can't say for how long. Prior to this encounter, I received the gardasil 4 vaccine. My questions are
1. Was this considered safe sex and what was my risk of acquiring HPV. Reading other people's post and from memory I saw that Dr Hook said there is virtually no risk of acquiring HPV through receipts of oral sex? Would you say that there is no risk for HPV acquisition then?
2. Since she was a low end prostitute, does this increase my chances of acquiring HPV from this encounter? This is what has especially made me worried. I really would not want to spread it to a future partner just because of this stupid decision by me. I am a fool.
3. Have you ever seen someone acquire HPV from a single encounter of oral sex (even without a condom).
I am going to see a dermatologist in a few weeks to see if my skin is normal but for now I want to at least potentially relieve my anxiety if I can. I always thought that this single encounter would definitely lead to me acquiring HPV if she had oral HPV and I'm assuming she definitely does have oral HPV because of her job.

Welcome to the Forum. Thanks as well for reviewing other posts on our site- we make them available to that folks can do what you just did- review and learn from them. Let me also make a few preliminary comments about the very low risk encounter you describe. First, having received the HPV vaccine, you can be confident that you have strong protection amongst the most common types of HPV including HPV 16 which causes most oral HPV-related cancers. In addition, HPV infections are far less common in the oral cavity than in the genital regions and condoms reduce the risk for infection. Putting all of this togther, your risk for infection from your single exposure is quite low. As for your specific questions:
See my comments above,. This was a minimal risk exposure which I urge you not to worry about.
2. Since she was a low end prostitute, does this increase my chances of acquiring HPV from this encounter? This is what has especially made me worried. I really would not want to spread it to a future partner just because of this stupid decision by me. I am a fool.
There are no specific data to inform my response to this question. HPV risk increases with numbers of partners and any commercial sex worker, low end or high end is likely to have had HPV, as a more adults who have had multiple partners.
3. Have you ever seen someone acquire HPV from a single encounter of oral sex (even without a condom)..
No
finally, as you check yourself going forward and with the dermatologist, I should point out that as persons scrutinize themselves more and more closely, they tend to "discover" bumps and cutaneous irregularities which had been previously unnoticed. Most such genital lesions are NOT HPV related. I urge you not to worry about this very low risk encounter. EWH

Thank you so much Dr Hook for your response
Just a few more questions.
1. Just curious but how come in other posts you say virtually no risk but then in this response, you say very low risk encounter? Are they basically equivalent. Could you perhaps quantify this risk?
2. I'm also unsure of if the condom even was on properly. Does this change your answer?

3. I know this is a similar q to 3, but have you ever seen someone get HPV from only oral. (not just a single encounter but can be multiple)
Thanks and I apologise if my questions seem repetitive but I am just extremely regretful about this decision which is causing me immense stress.

4. Also I am quite worried about penile cancer. How quickly does HPV progress to penile cancer usually? I have noticed some tiny bumps that tend to go away quite quickly but then they reappear in a different location and I am going to get this checked by the dermatologist soon. Does this potentially sound like penile cancer?

1. I get tired of saying the same thing. Virtually no risk=Very low risk.
2. No And I suspect you are being paranoid and overthinking this. Do you think the condom was just lying there?
3. No. If you stop and think about it for a moment, this would be very difficult to prove. Most people are exposed in multiple ways during most encounters.
4. The lesions you describe sound nothing like penile cancer. No, cancer takes decades to develop following HPV infection. Most importantly, you have taken the HPV vaccine which is highly protective against penile cancer in persons with HPV. Cancer lesions would not come racist or grow slowly (over a period of months or years)

Much appreciated Dr Hook
3 weeks after this encounter, I experienced pain on my foreskin area when I touch it and even when I don't and it looks a bit more red than normal and it has persisted for a long time(around 2 years) up to this date. I went to get it checked out but the dermatologist said they don't see anything wrong with it. Regardless, I have a few questions still.
1. How long after an encounter will one begin to experience HPV symptoms if they have any. Is 3 weeks definitely too short? Is it possible this pain is related to HPV?
2. I read on this site that the penile cancer lesions/bumps etc would be noticeable and they do not go away/heal themselves? Is this correct? I am worried those tiny bumps may be penile cancer. (I am in my 20s if this helps. I understand the cancer usually occurs in the older population but I am still very worried)
3. Virtually no risk basically means no risk I assume? Is this correct
Sorry for the paranoia and thanks so much for your help. I really hope to put this behind me.

As you know, we provide up to three responses to each client’s questions. This will be my final response as part of this thread:
---
1. The idea that HPV causes symptoms is incorrect. Other than possible painless lesions ( warts) HPV infections are asymptomatic. The discomfort you describe does not suggest any STI. If the dermatologist said your foreskiin appears normal it probably is. You could pursue further with another dermatologist or see a neurologist regarding the discomfort. Again, not you describe suggests an STI.
2. This is correct. Penile cancer is a disease of persons over 40. If the bumps you noticed were cancer a dermatologist would recognize it.
3. Correct.
I hope that the information I’ve proviso helpful. Please don’t worry about STI. This completes this thread. Take care. EWH
