[Question #8388] Herpes

Hello. On April 4th I had sex with someone I trusted. Four days later, I noticed two ulcerated spots at the base of my penis that had a lot of clear fluid coming from them. I had a third pimple that I popped that also turned into a similar ulcer.  All three of these were separated on different sides of the penis. These spots didn’t hurt or itch at all. The female I was with later told me she had herpes but did not disclose prior to sex.

I immediately went to the doctor a couple days later and had a culture swab done. That test was negative. The next day I went to another doctor and had a second culture swab that was also negative. About 7 days later I had an IGM blood test that was negative. I had IGG blood tests conducted at 4 weeks, 7 weeks, 8 weeks, 12 weeks, and had a western blot conducted at 16 weeks. All were negative. I had another IGG blood test done at 6 months, which is also negative. I’ve used a mixture of different labs (quest, labcorp) to do these tests.

I’ve went down the rabbit hole with research and have read that some people seroconvert later than others, and that it can take longer than 6 months to show IGG. I was almost sure I had herpes initially, based on those facts (time of discovery, what she told me, and appearance of ulcers). but all of my tests have been negative. Nevertheless, I can’t get over the paranoia and anxiety and it’s eating me up!

Do I need to continue to get tested? Is there a 100% time frame for IGG seroconversion? Am I in the clear? What do I do? I am otherwise healthy, never had an STD before, and have had no other problems since then. It’s been 7 months to date of potential exposure. Please help

If you had a negative western blot at 16 weeks out and it was negative for both HSV 1 and 2, I would believe that and move on.  I don't think you need to do additional testing at this point.    The only concern I have is this:  have you taken any extended courses of antiviral medication?  By extended, I mean more than 2-3 weeks.  If the answer is no, I think you should believe the blot and move on, putting herpes worry behind you.

Thank you for your reply! No mam, I have never taken any form of antiviral medication before. 

 A couple days ago after writing my first question I noticed a small single pimple on my penis that also ulcerated about two days later, but healed within 4 days. It also wasn’t painful. I’m not sure if my thoughts are primarily paranoia due to the similar situations and causing my anxiety to increase. Maybe I am being paranoid. I was just curious in your expert opinion if you have ever seen anyone’s immune system not kick in and build antibodies before? Or could my version of the virus be so minimal that my system hasn’t responded to it? I don’t believe I have any immune problems but I know I normally keep colds longer than most people. 

I almost even considered getting a second western blot. Regardless, I have stopped having sex lol. Your opinion definitely means a lot to me. Thank you

So herpes wouldn't look like a pimple.  It would look like a water blister.  If you have concerns that the blot missed an infection (and that would be very very rare indeed) I think you should continue to have  any concerns lesions swab tested for a few more times to get reassured that these are not herpes.
