Hi Dr’s,
First I would like to say thank you for your continued help with determining what is going on.
With that being said - my fears of HiV have subsided as the 4 week AgAb test were negative.
However, I do want to see what’s going on now. As I stated before I tested 10 days after my exposure for chlamydia/gonorrhea from a urine test taken at the Dr office. The test came back negative for both and they cultured it and found no bacteria. I was still given doxycycline out of precaution because I did have a white penile discharge as well as lower abdomen pain and a very mild anal itch (nothing has ever been up there. I’m heterosexual). I took the full 7 day cycle and so did my wife (she missed one pill). It’s now been a little over a week since our last dose but I am still feeling lower abdomen pressure/pain. It feels like I’m full of that my bladder is full and I just went pee. I also still have the very mild anal itch and I get a weird jolt like feeling in my penis. I haven’t had a discharge again that I know of. My wife seems to be a little itchy down there as well. I don’t know if she has had any recent discharges. I don’t have cloudy urine and it doesn’t hurt to pee at all.
1. Any idea what this could be?
2. Should I get tested for trich?
3. Could this be a male uti and maybe I need different antibiotics? I have read you can pass uti bacteria to your partner.
4. I know you may think this is anxiety related but I don’t believe the symptoms are made up in my head. Something is definitely going on down there.

Also - lower abdomen pain is on right side and right testicle is sore/painful as well.

I'm sorry you found a need to return once again. However, we really cannot help further, especially since these questions have already been answered in your previous two threads.
1. See my original reply: You had NGU probably due to normal oral bacteria. Since your symptoms have cleared with doxycycline, you can be sure the NGU is gone. NGU from oral sex has no known long term complications and probably is harmless for affected men's sex partners. Your minor residual symptoms sound anxiety related, nothing more. Your wife's symptoms do not sound abnormal or concerning: probably nothing is wrong.
2. Trichomonas is not carried in the mouth and thus not acquired or transmitted by oral sex, and not a likely cause of symptoms like you describe.
3. Your normal urinalysis (or whatever test you doctor has done most recently) prove you do not have a UTI.
4. Your skepticism is typical. Most people with anxiety driven symptoms tend to disbelieve it, but that remains my opinion based on more than 4 decades of clinical experience. (The additional symptoms in your follow-up comment also are consistent with anxiety.) And genitally focused anxiety is exceedingly common in men who come to regret a particular sexual exposure or decision. (This entire discussion could be used as a perfect case to teach medical students about genitally focused anxiety.) FYI, I'm not a believer in anxiety creating symptoms out of the blue, but there simply is no doubt that anxiety and other psychological stresses can magnify trivial symptoms or even normal body sensations that otherwise would not be bothersome or not even noticed. I cannot diagnose anything from this distance, but I remain remain confident that's the best bet. And in all those years, I have never seen someone with symptoms like yours who ended up having any proved infection or any complications or serious health outcome. Keep working with your doctor if your symptoms persist. But there is nothing more this forum can offer you.

Thank you for following up.
I know my body really well and can tell when something isn’t right. I do trust that I had NGU and I figured the doxy would have cleared that. I haven’t had a discharge since the initial one. But I do still have the stomach pain. It feels like pressure and that I have a feeling of needing to pee even after peeing. It’s a weird symptom. I still feel dizzy and off balance as well which is very odd for me.
I also have had yellow whites of my eyes.
And so does my wife! Could this be the Ebv?
My bilirubin was normal level according to my dr (when I went for the first initial blood draw oct. 13th it was 1.2. When my blood was drawn on November 8th it came back at .9 which I was told all within normal range).
I know you state there is nothing more this forum can do and that’s fine. But I do need one last piece of closure/confirmation before I go…
1. Do you think I need to test for anything else?
2. If the stomach issues are just residual symptoms from the ngu will they eventually be cleared by my immune system?
3. Why would both my wife and I have yellowing on the whites of eyes and look “sick”? Anything sti related?
4. Were any of my tests too early?
5. If you were me would you be concerned of anything?

Your anxieties are influencing your perception of eye color. (Despite your disagreement with the concept, you remain a great teaching case about how anxiety can influence personal health perceptions!) But if you think they really are turning yellow, then you and your wife have jaundice, i.e. your bilirubin now would have to be significantly elevated, which I seriously doubt. But if you disagree, your only rational choice is to raise this with your doctor, who may (or may not) retest your bilirubin and other liver function tests. In the exceedingly unlikely event you really have liver disease, I am confident it would have nothing to do with your sexual indiscretion or EBV.
1. No, you need no more testing unless you choose to pursue the yellow eye business.
2. NGU does not cause "stomach issues" of any sort (except perhaps through the anxiety/emotional route).
3. See above.
4. No.
5. Only your mental health! But I'm not your doctor. If you remain convinced that anything remains physically wrong, discuss it with your doctor.
I remain confident this is all a big nothingburger with no significant consequences for your health or your wife's.
