[Question #8420] STI risks with CSW

Hey doctors I am a male in my early 30s in a long term relationship living in the Seattle area. Recently got a referral for csw from a friend and was pretty curious to try it out. However , I fear of passing anything to my girlfriend. Goes without saying i will always use condoms as a way of protection. If I stick with covered oral sex what are my chances of getting any sti including herpes, chlamydia, hiv ( this one worries me a bit more) and what about vaginal covered sex ,same worry about STIs above.

It is easy (and common) to assume that any commecial sex worker is at high risk for STIs. But there is wide variation in risk, with some being among the safest sex partners one could hope for; others not so much. Probably there is a correlation with price and style, with lower risk among escorts (expensive CSWs by appointment) and highest among streetwalker, brothel workers, etc. But all may be safer than, for example, a non-commercial bar or hotel pick-up. But in general, most escorts tend to understand the risks, choose low risk partners (e.g. mostly sexually inactive or monogamous men -- guys like you!). Many or most insist on consistent condom use and get tested regularly. Among such women, the frequency of HIV is believed to be very low.
The other main point is that you apparently are planning very safe sex. Even without a condom, oral sex can be considered safe sex: extremely low or even zero risk for some STIs, and for all infections a lot lower risk than unprotected vaginal or anal sex. And with a condom, fellatio (oral-penile contact) is truly zero risk. And since you're apparently not planning on vaginal or anal sex at all, even condom protected, it sounds like you're on a path toward essentially no STI risk at all. Assuming you stick with these plans -- i.e. only condom-protected fellatio, no vaginal or anal exposure) you will be at no risk for any of the infections you mention, or for any of the STIs you don't mention. And if you select a high-end (expensive) CSW partner, then even in the event of condom rupture or other condom failure, or if you succumb to possible temtation and have vaginal or anal sex, you will be at low if any risk for most infections.

Thank you so much for the timely response doctor I wasn't expecting a reply until after the weekend. Your answer is conclusive regarding covered fellatio. What about vaginal or anal with a condom is that also no risk. And indeed csw is appt girl that has a place in high end area of town.

Perhaps I misinterpreted, thinking you were not planning on vaginal or anal sex ("If I stick with covered oral..."). Assuming vaginal or anal exposure, of course the STI risks will rise. But with a properly used condom (no significant contact before putting it on) and no condom failure, for practical purposes, there will be no risk for those transmitted by genital fluids, such as gonorrhea, chlamydia, and HIV. But condoms are not especially good protection against those transmitted skin to skin (herpes, HPV, syphilis). Still, even for these the risk will be minimal. But then there's always the potential problem of condom breakage, which probably can be minimized by using a quality brand (as opposed to a vending machine purchase). Also, there is some belief that polyurethane condoms are less prone to rupture than latex ones, although I'm not aware of any solid data on this.
But it sounds like you're planning on a partner at the low end of STI risk, as discussed above. You might wish to discuss it with her before proceeding. She might be expecting such questions anyway.
