[Question #8462] Unprotected oral with csw - small cuts on penis

Hello experts,
I am an uncircumcised male in US. I get regular check-ups for STDs. Last time was 1 month ago and everything was negative. Today I had unprotected oral and protected virginal sex with a high-end (500USD/h) CSW from Australia who does the job to fund her college education. She is originally from China and she said she gets 'regular GYN check-ups' in Australia although she 'did not try to do any tests only for STD purposes'. She also said she had never ever had sex without condom, even with her ex-boyfriends. Based on our interaction, I believe her.
While the unprotected oral was happening, I could feel some itchy/painful sensation from my penis, and I can still feel it now, although I could not find any visible cuts. So small cuts are very possible. I indeed had fragile frenulum because of yeast infection, but it was gone after antifungal cream was applied and it never came back. The virginal sex was protected and condom did not break. I could have touched some of her body fluids for sure - just like all others with sexual conducts. Considering all of these facts, does your stance that 'HIV is no to minimal risk' stand? Should I be concerned with anything? I understand the some bacteria infections are possible, but since they show symptoms and are easily curable with antibiotics, I am not that concerned.
If you were me, would you, like your other answers, 'have sex with my regular partners without getting tested'?
Thank you!

Welcome to our Forum and thanks for your questions. I'll be glad to comment. The encounter you describe was very low risk. Your only (small) risk for STI from receipt of unprotected oral sex was for acquisition of gonorrhea and, as you note, gonorrhea typically becomes symptomatic within 3-4 days after sex. Your vaginal exposure was condom protected and therefore safe sex. i would not worry and see little medical reason for concern or testing.
The discomfort you experienced may well have been related to friction. As you note, you may already be a bit susceptible to friction due to the yeast infection you mention.
I would not be concerned about STI if I were you and would not hesitate to have unprotected sex with my regular partner.
I hope this perspective is helpful. EWH

Hi Dr, Hook,
It is definitely helpful. Thank you very much. Since I do have 2 opportunities to ask follow-up questions, I would like to let you know that I indeed just saw ONE very tiny red spot (raises a little above other skin) on my foreskin close to the shaft. It is NOT itchy nor painful. It is so small that I can't tell whether there is any liquid or pups inside. I am also having a bit discomfort: when I try to touch my penis, I could feel some itchy/painful sensation from my penis. Still, no visible cut found and I can't tell where exactly the sensation is from. I (short unprotected oral and protected vaginal sex) with the same CSW 4 days ago. I have no other symptoms. (no discharge, no burning sensation when urinating) Any thoughts? blantitis? Thanks!

Often we find that when clients become concerned about STI/HIV risk following an encounter that they have come to regret, they tend to inspect their genitalia far more closely than ever before and, in doing so, come to notice normal sensations, small lesions, normal urethral discharge, and other irregularities which were previously unnoticed. I suspect that this is what is happening here. I suspect that this is the case here. In some instances of this sort, clients choose to test for the most common STIs (gonorrhea, non-gonococcal urethritis (NGU) for the peace of mind it provides. The testing virtually always shows that no infection is present. I would not worry but if you choose to test, please commit to accepting the results, which I anticipate will show no infection, when they are available. This in no way changes my earlier assessment. I would not hesitate to have unprotected sex with your regular partner at this time. EWH---

Thank you! To summarize: if I always have condom-protected vaginal sex (and make sure I use it correctly, and it does not break), and unprotected oral sex with female csw, the probability of me getting HIV is close to 0 when the absolute 0 can not be achieved. Probably equal to: hit by lightening, contracted some unknown virus, hiv becomes mutant and can be transmitted through air, etc.....
