[Question #8481] STI risk with active pornstar

Hey Dr this is a slightly different question than my previous post about CSW. Recently came up with the chance of booking a session with famous pornstar and was trying to get your professional opinion on the possible STI risk including hiv , gonorrhea, herpes , etc. This pornstar provides uncovered oral and protected vaginally as well as ejacultion in her mouth. Assuming I partake in all the activities above do I need to get tested before engaging in unprotected sex with my regular partner?
Additionally pornstar can provide proof of testing for further re assurance. Let me know your overall opinion as this is probably something I would only do once in a lifetime.

Welcome back to the forum. On this occasion I happened to pick up your question rather than Dr. Handsfield. In preparing to reply I reviewed your earlier interaction with Dr. Handsfield and agree with all that he said. This new question is really a version of your earlier question, differing only in that receipt of oral sex would now be unprotected rather than condom protected.

In general there is a modest risk of acquiring an STI from receipt of unprotected oral sex. Oral STI’s are less common than genital infections and oral infections appear to be somewhat less likely to be transmitted on any single exposure then when exposed through genital contact.  The STI’s acquired most commonly through receipt of oral sex are gonorrhea and non-chlamydial NGU which is a Urethritis (penile irritation presenting as a discharge or discomfort on voiding) which is often considered non-infectious to partners. Chlamydial infection is very, very rarely acquired from oral sex. If you have not already had HSV-1 (cold sores, statistically most likely you have already got HSV-1)  there is a very small chance of acquiring genital herpes type one from oral contact as well.  There is no known risk of acquiring HIV from receipt of unprotected oral sex.

That your potential partner works in the adult film industry has little impact on the risk and, if anything, since she gets checked regularly, may be somewhat lower than average. That your prospective partner gets checked regularly, as is often required by persons engaged in the adult film industry, offers added protection. Obviously how recently testing was performed will determine how likely it is that infection has occurred in the meantime although as a generalization, previous testing makes infection less likely.

Obviously, as a generalization, after engaging in unprotected sex of any sort following an encounter with a new partner, getting tested is the safest course of action. If you need testing, the most important test for you to have performed is a urine test for Gonorrhea. I hope this information is helpful. EWH

Dr would i also need testing for gonorrhea if I stick with covered oral and covered vaginal?

As long as the condom is worn through the encounter and does not fail, there would be no scientific reason for testing.  (FYI, when condoms fail, they break wide open leaving no doubt that they have failed.  No need for "water testing" or concern that they might have leaked without an obvious tear.)  EWH---

In case I do succumb to unprotected oral , can hsv2 and gonorrhea manifest in a asymptomatic way or I would notice  within a period of 1 to 2 weeks?

These infections are most typically symptomatic and would become symptomatic within 10 days of exposure. Personally, I would not be very worried. EWH---
