Dear doctors,

I really did not want to come back here, but unfortunatey came back.

Last year, I have had dozens of sexual contacts in many massage parlours.

Most of this contacts are sexual massages including hanjob except one brief insertive oral sex and one fingering. No penetration on vagina, anus, not at all.

The oral sex was done in last year September and the fingering was on November.

And my last sexual event with handjob was Dec. 18th, last year. At that time I touched the masseuse's genital area on the pants and then masturbated myself with the same hand.

After 8 days of last event, I started to feel that  I got a cold with severe sore throat. And the next day, I felt some neck pain with lymph nodes hurts. I had some painkillers but the symptoms were not relieved.

I visited two ENT specialists and they said that my lymph nodes on left neck and under the chin were swollen. And those were quite painful when pressed. And my throat was also swollen a lot. Cause I had removed my tonsil, the swollen area was the back side of throat.

There was no fever or rash or muscle pains but I felt extremely exhausted for 4 days and some joint pain for one week.

The symptoms have gotten better day by day, but my lymph nodes on my neck is still swollen for more than 18 days and still tender. And has not shrinked significantly.

And during these symptoms my lymph nodes in front of my left ear was also swollen only for two days and subsided.

So I wonder these kinds of symtoms would suggest ARS or not.

Welcome back to our forum. I’ll be glad to come at. Although you have had many encounters, None of them are the sorts of contacts that have ever been proven to be associated with acquisition of HIV. Most masseuses and commercial sex workers do not have HIV. Even if one or more of your partners did, receipt of masturbation, masturbating a partner ( fingering), and receiving oral sex are all no risk activities.

I suspect that your sore throat was coincidental and could’ve been caused by any number of common, every day throat infections including infections due to strep add many, many types of viruses. When throat infections occur, it is typical for the lymph nodes of the neck to swell and become tender as part of the illness. The swelling can remain present for long periods of time.

With the ARS occurs the sore throat and lymph node swelling is typically accompanied by high fevers and severe muscle or joint aches.

Given the nature of your contacts, I am confident that the symptoms you describe are not due to the ARS. If you want to prove this to yourself, a fourth generation combination HIV antigen/antibody test would be positive at this time if your sore throat and related symptoms were due to HIV. Personally, I see no reason for testing but if you feel the need to do so too relieve your concerns, the test would be positive if your symptoms were due to HIV.

Thank you very much for your answer.

So there is absolutely no risk of HIV on any sort of mutual masturbation without penetration regardless of the existence of secretions, Am I right?

And I have another question about symptoms. Do ARS symptoms start all at once? Or can those start at different time?

My left armpit started to be hurt and tender 4 days ago and this is 2 weeks after my initial sypmtoms onset. I cannot feel any lump but it is tender.

So during the ARS, do the lymph nodes in the body start to swell at the same time?

And unfortunately, I am not in my home country now, and this country revoke the visa and deport the HIV positive person. Do you think that I can test my status without any concern of deportation?

1.  Correct, there is no risk of HIV acquisition from mutual masturbation, even when participants get each other’s general secretions on one another.

2.  Yes, the symptoms of ARS typically occur all at about the same time. Further, the lymphadenopathy which occurs as part of the ARS is typically asymptomatic. Your lymph node swelling, both in your neck, and later, in your armpit. Do not suggest ARS.

Your concerns about your visa are unfounded. The exposures you described are no risk and should not concern you. If you test for HIV, your test will be negative. I am confident of this. EWH

This is my last question.

You mentioned that ARS lymph nodes swelling is typically asymptomatic, does this mean that the lymph nodes during ARS isn't hurt or tender?

The lymph node swelling associated with the ARS may be tender as the syndrome begins but then the swollen nodes may persist painlessly afterwards.  Because there are so many causes, lymph node swelling is an unreliable indicator or HIV.
