[Question #8537] Regarding ongoing symptoms after a exposure

Hello doctors i recently had an sexual exposure with csw in australia with asian women and fortunately i used condom and what happened is she gave me blow job with the condom on for few seconds and then i inserted my penis into her vagina just for 2 to 3 seconds and then removed that condom and had another new condom and again she gave me oral sex on condom and in all the encounters condom is present and after all these as its a dark place i dont know whether the condom got broken or not but i didnt see any noticable fluids on my penis after all these encounters and within 13 minutes after the encounters i washed my penis thoroughly but didnt find any liquids at that time as well. And the encounter is on 12th january and i felt dizziness and headache on 4th and 5th day and nasal congestion and sneezing and on the 6th day i started feeling tingling and pins in my left leg starting from the hip and down to the feet and its more when i am sitting or sleeping i feel those sensation. i tested for covid and its negative.
my questions are
1 - if at all there is a condom break and whats the chance of getting infected to hiv and other stds for 2 to 3 sec insertion and removed that condom immediatley ?
2 - Is tingling and pins in the feet and left leg the symptom for hiv and it started on 6th day after exposure and still experiencing since 2 days ?
3 - Does acute hiv symptoms start on 6th day and have you ever seen any case like that?
4 - have you ever seen this symptom of tingling and pins in legs in acute hiv stage?
5 - overall do you recommend me to go for testing and whats the chance of me getting infected with hiv?
6 - Does condems provide 100% protection from stds if not broken?

Welcome to our firm and thanks for your question. Congratulations as well on your commitment to safe, condom protected sex. It does much to nearly completely eliminate risk for acquisition of HIV and other STI’s. The symptoms you report are not suggestive of recently acquired HIV. Nasal congestion is not part of the syndrome which accompanies recently acquired HIV, nor is a pins and needles sensation of the extremities. Testing for Covid was a good idea and your negative tests makes it unlikely that that is the cause of your symptoms. A number of other common, every day viral respiratory tract infections however can cause the sorts of congestion and headache that you mention. In answer to your specific questions:
1. It is unlikely that your condoms broke. When condoms break, they break wide-open leaving no doubt that they have failed. Regarding your question about the impact of duration of exposure on risk for HIV, there are no scientific data to quantify this however logically, the longer the exposure the higher the risk. Thus a 2 to 3 second exposure would be quite low risk.
2. See my comments above. The tingling sensation you describe is not a sign of recently acquired HIV or any other STI.
3. As mentioned above the symptoms you describe do not suggest recently acquired HIV. The onset of symptoms five or six days following exposure would be early side for the onset of symptoms due to recently acquired HIV.
4. No
5. From your description, I see no medical reason for HIV testing although if you chose to test for your own peace of mind, a negative test result would prove that your symptoms are not a result of HIV.
6. It is very challenging to study how well condoms work since self-report is the main way condom use is measured. Perhaps related to that reason, even in large, carefully conducted studies, there are very rare instances in which persons who state they have used condoms acquire STI’s. Since condom use can only be measured by self report, many investigators wonder about the accuracy of self report in those cases. In general however condoms are more than 99% effective for reducing risk for STI’s, including HIV, when worn throughout the encounter and without breakage which occurs about 1% of the time condoms are used.
I hope the information I have provided is helpful. I would not be concerned about HIV from the exposure you report. EWH

Thanks for your reply doctor, I am still experiencing that tingling mostly in the left leg few times a day usually in afternoon and i found two to three red spots not at the same place but one on hand, leg and one on stomach feels itchy and raised above the skin but they appeared only when i scracth on that area and after 1 to 2 hours they disappear and i no longer felt itchy the next day they disappered
1. do you think are these acute hiv symptoms
2. can i move on with my life considering these are not acute symptoms or do i need to test

. I remain confident that your symptoms are not due to HIV. if you wish and are unrelated to anything you might have caught from the encounter you described. If you wish to prove this is not HIV, test. If the symptoms were due to HIV, the test would be positive.
2. I would encourage you to do just that.

Hie doctor today i had Abbot alere 4th generation ag/ab rapid hiv test at 47th day after exposure and it came back negative and i even had 4th generation labtest and waiting for the results
1. How accurate is 4th generation rapid test at 47 days
2. Can i expect the same result which is negative in lab based fourth generation test
