[Question #8626] Condom Slippage While Withdrawing

Hello doctors! I’m sorry to be back but I just wanted to make sure of my HIV risk.
Last night I was fooling around with this guy. It was fairly straightforward. I wore a condom and I topped him. At no point while I was thrusting my penis came out of his ass. After ejaculating (while inside), I proceeded to thrust in a bit more. But when I withdrew my penis the condom was left inside him. Now I’m thinking my penis must have been lubricated with my semen causing it to slip off. He pulled the condom out of him and I visually confirmed that I came inside the condom.
What are my risks for HIV? I think I am at a very low risk but can I consider the whole event to be compared to protected anal sex? Thought of asking this question here since most of the questions I found online were penile-vaginal sex.

Welcome to the Forum and thanks for your question. I'll be glad to comment. In general, when there is discussion of condoms slipping off during sex most references pertain to vaginal sex however the mechanics are the same- following ejaculation, the penis becomes a bit less firm making it possible to slip out of the condom upon removal. As long as the opening of the condom is outside of the vaginal or rectum, the condom should have done its job as protection. It is reasonable to consider this as safe sex.

Thank you for your reply doctor. It’s been reassuring. Final follow up question, based on what I’ve told you, does it warrant a DUO test after 28 days?
I should add that the guy said he always packed condoms whenever he was going to hook up since he was also being very safe. I assume this also reduces my potential risk.

I may also have to add that I think I can be confident that the opening of the condom is by the rectum since I never withdrew my penis up until ejaculating!

Thanks for the additional information. The interaction you describe is relatively safe sex and you were kind of projects protected. I don’t believe it is urgent for you to test related to this encounter. Obviously, should you choose to, the results may be reassuring to you. Results of DUO testing at 28 days will be well over 95% conclusive and at six weeks will be absolutely conclusive. Should you choose to test, I anticipate the results will show that you were not infected.EWH---

Thank you doctor for your answer! Though I still have one more follow up question, I think you may close this thread now. Maybe I’ll take a DUO test in 28 days for the peace of mind!

Thanks for your comments. My final comments have less to do with the encounter we discussed than the future. I support your inclination to test at 28 days, mostly for the reassurance it will provide, not because I’m worried that your risk is high. In the future:
1 Continue with your commitment to safe, condom protected sex. It will keep you safe.
2, I encourage you to ask future partners about recent testing, their HIV status, and when they were last tested. Most people tell the truth
