Hello, please note following details:

2017 - I had multiple sexual exposures (unprotected vaginal sex)

December 2017 – My last sexual exposure before I got married (unprotected vaginal sex, I’m male, I was 20 years old in 2017)

April 2018 – I got married and started having unprotected vaginal sex with my wife (I did not yet test for HIV and Syphilis, I was 21 years old in 2018, my wife was 25 years old female in 2018)

September 2018 – I got tested for HIV (4th generation HIV ELISA test, I was 21 years old in 2018)

November-December 2018 - I got tested again for HIV (4th generation HIV ELISA test) and for Syphilis (RPR, TPHA and EIA Syphilis tests) - I was 21 years old in 2018

All test results were negative. I am from India, my wife is also from India. Let me know if you need any other detail.

Please clarify:

1. My wife never had any other sexual exposure except me. My wife never had any test for HIV and Syphilis. With above details, can I be 100% sure that I NEVER HAD HIV and Syphilis? I want to make sure I never had HIV and Syphilis and hence there is no way I could have transmitted HIV and Syphilis to my wife. 

2. Syphilis TPHA and EIA test can serorevert from positive to negative if the Syphilis infection is treated in its primary stage. Could Syphilis TPHA and EIA test serorevert (from positive to negative) if someone is treated with antibiotics after 4 months of Syphilis infection?

3. If I was taking antibiotics during Syphilis test and few days before Syphilis test, will antibiotics interfere with Syphilis test (RPR, TPHA, EIA) causing false negative test result for Syphilis?   

4. I do not remember all medications, I might have taken medications for cold, flu, eye stye, etc. at different intervals between my exposures and 4th generation HIV ELISA test. Can any of these medications interfere with 4th generation HIV ELISA test causing false negative test result for HIV?

Welcome to the Forum and thanks for your questions.  I'll be glad to comment:
1.  Correct.  Your test results conclusively indicate that you had neither syphilis nor HIV.  If your wife has never had sex with anyone else and has not been injected with contaminated material, she would not have acquired HIV.

2.  Seroreversion of the sort you describe is rare and typically occurs only when persons are treated very early (within weeks) of acquiring syphilis.  Seroreversion only occurs when person are treated early in the primary stage of syphilis.  Treatment at four months would not lead to seroreversion.

3.No

4.  No medications cause false positive HIV test results.  Believe your tests

Thanks for the reply, doctor. Since English is my second language, kindly let me ensure I understand your responses correctly. I would like to confirm the following:

1. My wife and I are together. I did not want my wife to have unnecessary tests for HIV and Syphilis, that's why I reached out to you who are the top experts. My wife never had sex with anyone else except me, she never injected drugs, etc. Thus, my wife never had any exposure to HIV and Syphilis, my wife only had unprotected vaginal sex (multiple times) with me. Since I tested negative for both HIV and Syphilis (in December 2018), I can be 100% confident that I NEVER HAD HIV (Type 1 and/or 2) and Syphilis at any point of time and hence, could not have transmitted HIV (Type 1 and/or 2) and Syphilis to my wife. Kindly let me know if my interpretation is correct.

2. Since I am from India where there are few cases of HIV-2, will all the points in your reply and the above summary by me be applicable to HIV-2 as well?

3. Is it possible for 4th generation HIV ELISA test (HIV Duo ELISA test or HIV Antigen-Antibody ELISA test) to serorevert from positive (positive for HIV-1 and/ or HIV-2) to negative in any circumstances?

4. In serial number 4 of your reply, did you mean “No medications cause false negative HIV test results.  Believe your tests”?

Glad to confirm:
1.  Your interpretation is correct.
2. Yes. My comments are applicable to HIV-2.
3.  No, once a person is infected, their tests will remain positive for life.
4.  There are no medications which cause falsely positive or falsely negative test results.

Thanks doctor for your reply. The only reason I was worried was because I tested for HIV (1 and/ or 2) and Syphilis after I had already started having unprotected vaginal sex with my wife. However, with your reply, I have understood that it is 100% conclusive that I do not have and never had HIV (1 and/ or 2) and Syphilis, and hence, there is no chance of transmission of HIV (1 and/ or 2) and Syphilis to my wife. Thanks a lot for your help.

Just final follow up questions:
1. I have read that some infections like Syphilis, Chlamydia and Gonorrhea can be cleared by the immune system. Is HIV (1 and/ or 2) ever cleared or cured by the immune system or anthing else?
2. Is negative 4th generation HIV ELISA test (HIV Duo ELISA test or HIV Antigen-Antibody ELISA Test) 100% proof that one does not have HIV (1 and/ or 2) at present and never had HIV (1 and/ or 2) in the past?

Final Responses:

1.  There are two or three cases in which HIV was cured with bone marrow transplantation.  There are no data to suggest that the immune system would clear the infection on its own.
2.  Yes.
