[Question #8654] HPV or Molluscum

Hello Doctors,
I want to start with thank you for this incredible god send service.
My situation started about 5 months ago. Iam a male 33 had a one night stand with a female same age I met in a restaurant. We had sex twice in the same night protected, but the second time when I pulled out after ejaculating the condom slept off and was stuck at entrance of her vagina. I could feel the condom at least was covering the tip of my penis before I pulled out. She assured me she was clear of all stds and I shouldn’t worry as she has been sex free for the entire year.
About 6 to 8 weeks later my testicles started to itch and found three hard white bumps on the right side of my scrotum. These bumps were not in contact with her vagina as they are located in the middle right side of my scrotum. So no friction. I went to a Health clinic and the nurse showed me some pictures, and I thought they looked like the hpv ones because they were white. So we decided on hpv without using a magnifier to look closely. She just looked with her eyes. She gave me Condylox to apply twice a day for 3 days followed by 4 days off. So I did use the gel and the bumps were gone in 5 days.
My questions are.
1 Before I used the gel I took pictures and when I zoomed in these bumps they had a white densation in the center, and when I removed the top skin with my nail I saw these white dots similar to what Molluscum core looks like, in fact pretty identical.
2, if these bumps were hpv could they be gone within a week of treatment with Condylox, I only used it for 3 days.
3, can Molluscum grow on top of hpv warts? Im saying this because after the treatment the white hard skin fell off revealing flesh colored lesions that bursted blood.
I am somehow certain I was misdiagnosed, and don’t know what to believe my eyes or the nurse. Iam currently in a different state for work and going to be with my wife in 3 months. So far I have been almost 5 months clear of these lesions and by the time I see my wife I w

Welcome to our forum. Thanks for your question. I’m afraid that giving you a precise answer will be challenging. Obviously, making a specific diagnosis on the basis of a verbal description or even pictures is more than difficult not to practice on this forum. I must say that I’m a bit troubled by the idea that the clinician who saw you out the need to review pictures with you regarding possible diagnoses.
Regarding your symptoms, Itching is a non-specific symptom which is, quite frankly, is rarely associated with an STI. The lesions you describe could very well be the sorts of benign cysts that normally occur on the scrotum. They certainly sound that way. On the other hand it is hard to deny that there is a possibility a that the lesions are due to molluscum or, somewhat less likely HPV (having the onset 6 to 8 weeks following an exposure would be quite rapid for HPV). The very rapid response to Condylox is a bit unusual for genital warts what is a possibility for molluscum. Thus, in answer to your specific questions:
1. I understand, this description makes it somewhat more likely that you are lesions were either Moscom or normal scrotal cysts.n
2. What is mentioned above, the rapid response is somewhat unusual for a genital wart.
3. Co-existence of molluscum lesions and genital warts would be someone unusual and, as I’ve mentioned the time course of your experience is relatively rapid for HPV infection.
To summarize, you may never know precisely what these lesions are. Irrespective, the fact is that apparently deletions have now resolved and have not recurred. My advice is for you to put this episode behind you and move forward without continuing concern. I see no need for additional testing or therapy. EWH

I found another picture and compared it with Molluscum lesions online, they were very identical in that they both had a dimple in the center. Do you think warts have that too?
When I removed the dimple with my finger nail I saw some white stuff inside the core of the lesion. Doesn’t that assure me is Molluscum?
I read one comment by Dr Handsfield where he said it’s almost impossible to remove the first layer of a wart without blood and pain. What’s your take on that?
Also, do you think the location of these bumps is unusual as they are right where I like to scratch my self in the right side of my ( balls) excuses my language. As there was no contact with that area.
I cannot sleep anymore Dr Hook fearing I will infect my wife. Next time I see her I will be 8 months free of any lesions. How should I proceed.
Should I just risk our marriage and tell her I had an hpv infection and that she should be vaccinated.
I’m torn apart, even though I’m almost certain now that its Molluscum, and not hpv
Based on all these descriptions. What it is more likely this to be in your opinion.

Your description is certainly more compatible with molluscum or a cyst rather than a wart ( which would most likely bleed upon being scraped off). As I said above, the rapid responses also more compatible with meloxicam.
After being free of lesions for eight months you could be quite comfortable that it would be unlikely to transmit that infection to your wife.
I sincerely doubt the lesions you describe were warts. EWH

Sorry I forgot, you said it’s unusual for hpv warts to be underneath a Molluscum lesion. Is it even possible for this scenario to happen. The two coexist in the same exact spot? Have you ever seen this in your decades of experience?
