[Question #8673] Urethra Irritation

I’m a male and received unprotected oral from another male on Jan 2nd. I’m married and this was a one off experiment I won’t be trying again. It was a brief encounter that lasted about 2-3 min. 17 days later I noticed the tip of my penis around my urethra opening became sensitive to my underwear. It would intermittently become red and one side of the opening would look swollen. Some days I wake up and the swelling looks better, some days worse. Sometimes the look of inflammation changes throughout the day. I’ve been experiencing this for the last 58 days. I’ve had no discharge, no pain while urinating, no hesitancy, just this feeling like the head of my penis is extra sensitive. I’ve checked daily for sores and have never had any show up on my shaft or head. I did see a small red spot on the inside of the urethra on Feb 19th but I had to look really hard. Dr. HHH always said you shouldn’t have to look that hard. Of course I’ve been panicking ever since. On Jan 25 called the doc, he prescribed me 2G of Azithromycin. Didn’t seem to help at all, so I went in for labs on Feb 15. (6.5 weeks) Ghon, Chlam, HIV, Syphillis, HSV-2 IGG all neg. Was positive for HSV-1 IGG but I already had a history of cold sores as a child and previously tested positive so wasn’t surprised by that. MD took a look at my urethra, rather briefly, and said it looked fine. (Slightly concerned because he wasn’t real familiar with NGU so not sure how great his assessment skills are). I’ve read hundreds and hundreds of questions on medhelp and I’ve seen that prior HSV-1 infections make me immune to a new infection anywhere else on my body and that HSV-2 is also zero risk. So how likely am I able to catch either one of these? I’ve been freaking out for 74 days straight. I know by 6 weeks post exposure my HSV-2 IGG should be around 80% correct. Is this CPPS,nerves? My wife and I have been having protected sex as to not get pregnant but she wants do now switch to birth control. I’m freaking out.

Welcome to our forum and thanks for your question including the detailed description. I’ll be glad to comment. Discomfort or increased sensitivity at the tip of the penis in the absence of visible symptoms other than possible swelling is not a typical symptom for any STI. Further, the absence of any response whatsoever to the azithromycin you took also suggested this problem is not due to a typical STI. Your negative tests, taken after you took the azithromycin provide further evidence that there is not an active infection present. There is certainly little or no evidence that this might be a manifestation of herpes. I do not have a good explanation for your symptoms but I am confident that they do not reflect the presence of an STI. I would not worry about having unprotected intercourse with your wife.
Indeed, this may represent CPPS or increased vigilance on your part. The other process that comes to mind is that occasionally people with prostate infection or kidney stones experience pain limited to the tip of the penis.
Sorry I don’t have a better explanation for your symptoms. I remain confident that this is not a typical STI and see no reason for continuing concern. If your symptoms persist you may want to seek evaluation by a urologist for other possible Casas. I hope this perspective is helpful. EWH

I appreciate your timely response. I guess I’m more concerned about HSV more than anything at this point. Based upon the timeline I gave, is it all possible that I have HSV-2? Have you seen someone with daily penis discomfort for 58+ days that turned out having some form of HSV? I know you doctors have said that if you have HSV-1 you’re immune to a new infection, but how often has it happened where someone got a new infection? Have you seen it? What are the odds of acquiring HSV oral to genital? I know I’ve read countlessly that it’s rare but what are we talking about? Do I have a better chance getting hit by lightning? My wife has had vertigo for the last two days and I’m worried I gave her something. Have you heard of anyone getting that with an initial outbreak? Sadly I’ve been through this once before long ago, and it was all in my head. I’m really hoping this is the same but even after reading your reply I’m still on edge. Should I get tested again or do you think I just need to call myself down?

Straight to your questions:
HSV almost never occurs in/on the oral cavity and as such the risl of acquiring it from receipt of oral sex is virtually zero. Further, discomfort of the duration you have experienced it from genital herpes is unheard of. Herpes is simply not a possible explanation for your symptoms.
My sense is that you are severely overthinking this and now looking for any abnormality in you or your wife, then trying to attribute it to your misstep. This is not productive. I urge you to put your concerns aside and move forward. There is no need for continuing concern or for additional testing of any sort. Really!! EWH
