[Question #8678] Chlamydia/gonorrhea test/lab reliability

Hello,

I am a 22 year old male and I had unprotected vaginal sex with a woman who was vague about her testing history. I had no symptoms, yet still wanted to get tested. So, I ordered a chlamydia/gonorrhea test from LetsGetChecked, a company specializing in at home testing. They are partnered with United HealthCare and CVS. The test simply consisted of providing a urine sample and mailing it to their private lab, Priva Path Labs, which is a CLIA certified and CAP accredited lab (CLIA #: 05D2180792). Their testing materials are made in an ISO accredited lab. I have become aware that in some states Planned Parenthood performs this as well, where they have patients mail in their sample, so I trusted this process. I waited two weeks to provide the sample, and the urine was from my first pee in the morning. They performed an FDA-approved NAAT test on my sample, titled HOLOGIC APTIMA Combo 2 Assay. The test result came back negative, and overall I consciously trust my test and am not worried. However, I have some nagging doubt due to issues with anxiety. I am wondering if I can simply and safely move on with my negative result?

Welcome to the forum. Thanks for your confidence in our services.

As you seem to have learned from your very extensive research, you have had the gold-standard and most commonly used test for both gonorrhea and chlamydia. You waited longer than necessary; negative results are valid any time more than 2-3 days after exposure for gonorrhea and probably 4-5 days for chlamydia. You indeed can move on with confidence you don't have either of these STIs.

Thank you, that is very helpful. I have one follow up question:

6 weeks afterwards, I also went to a clinic and got a blood test (from the vein) for syphilis and HIV. The HIV test was an antigen/antibody test. Both tests came back negative. Are these tests conclusive after 6 weeks?

Your HIV test result was conclusive. The antigen-antibody (AgAb, "combo", "4th generation") HIV blood tests have been discussed extensively on this forum, at least one or two questions most days. Negative results are nearly conclusive 4 weeks after the last possible exposure and 100% conclusive at 45 days (and 6 weeks -- 42 days -- is close enough).

Syphilis -- probably conclusive, but (surprisingly) there are not conclusive data on this. Some experts point out that it might take 8 or even 12 weeks for some syphilis blood tests to become positive. On the other hand, heterosexually transmitted syphilis is rare in most clinical settings, and infected men usually develop an obvious chancre (syphilitic sore) if infected. So the combination of low risk, no symptoms, and negative blood test at 6 weeks can be considered conclusive.

Thank you those answers are very helpful. I know I have one more question left then the discussion will be closed.

The night before the blood tests I had two alcoholic drinks (beers), but it was around 15-16 hours before my test so the alcohol would've been out of my blood stream by the time I took the test. I am wondering if this would have any effect on tests? I am assuming this is an irrational worry, but just wanted to make sure because I saw somebody asking about it on another post.

No medical conditions, medications, or drugs (including alcohol) have any known effect on reliability of the standard tests for HIV or other STIs. No worries at all!
