Hi doctor for the past few years i’ve been to high end escorts either independent or with agencies In Canada. now my sex has always been protected with condom even for oral sex. three questions i have, with some of the escorts i perform oral doing (69) is that any risk of hiv? (i had condom
on me) when she was giving me head.
Secondly, with one of the escort i was having intercourse at one point i notice my condom slipped without any ejaculation is that any risk of hiv?
Thirdly,  sometime with escorts i play with their Vagina or i insert my finger into their anal i do that because it gives me good feeling please don’t (judge me) is that also any risk of hiv?

this test i did im not sure if its std test or not my parents told me to take a test and im afraid if its std test because its says hiv on it can you tell me if this is std test or not also? thanks. sorry if im posting image link not sure thats again'ts rule website.

Welcome to our Forum and thanks for your questions.  I'll be glad to comment:

Before I get to your specific questions, let me point out that most CSWs, particularly high end ones, rarely have STIs or HIV.  They tend to be good about testing and risk avoidance.  Further, most exposures to infected partners do not lead to infection.  With this background, let's address your specific questions:

1.  Even if your partners had HIV (unlikely), oral sex is a low risks activity.  There are no instances in which someone has been proven to acquire HIV from receipt of unprotected oral sex from an untreated, infected partner.  Cunnilingus is low risk and inefficient for HIV transmission. There are a handful of cases in which HIV may have been acquired from cunnilingus but they are so rare as to not permit accurate estimates of risl  The best estimates suggest that IF your partner had HIV, your risk for infection would be less than 1 in 10,000, i.e. changes are 99.99 that you would NOT be infected.  I really would not be concerned about the exposures you describe.  

2.  if your condom slipped down but did not slip completely off, it would still provide protection.

3.  The fact that your partners may masturbate themselves (or you them) during your encounters does not put you at risk for HIV or other STIs.  Masturbation, even hen partners are exposed directly to each other's genital secretions is a no risk event, i.e. safe sex.

I glanced at the picture you provided (we do not look at clinical pictures) and see no tests for STIs.

I see, in general oral is it risk of hiv at for example french kissing to oral cunnilingus or rimming any act of oral? if i decide to take hiv twst sti what would my result be?

As I suspect you know, we provide up to three responses to each clients questions. This will be my final responses as part of this thread. The thread will then be closed soon after I post this response call

There is no risk for HIV from kissing, including French kissing. Likewise, I am unaware of any cases of HIV acquired do to rimming.  On the other hand as I have already told you, there are a very, very small number of cases in which cunnilingus may have led to acquisition of HIV. This is a rare event and far, far less common than acquisition of HIV through genital-genital contact.

If you choose to test for HIV, results will be conclusive six weeks after your last exposure. If you test, I am confident that it will show that you did not acquire HIV from the encounter you describe. Similarly, should you decide to test for other STI’s such as gonorrhea or chlamydia with a throat swab, once again I am confident your results will show that you were not infected.  Cunnilingus is simply a very inefficient route for acquisition of any STI.
