hello doctor

This question is a follow-up to my previous question, as you know, I did a combo hiv test in lab on day 11 and it was negative, day 14 and it was negative, and on day 23 it was negative in addition to all sexually transmitted diseases that were negative, but yesterday, on day 26 of unprotected sexual intercourse, I had a fever. Surprise, it reached 38.8 degrees Celsius, accompanied by pain like pulling in the area of ​​​​the thigh, close to the pubic area, and pain in the back, but there is no cough, cold or anything else

My questions, Doctor:

1- Could these symptoms be the symptoms of Ars hiv? Where I read on some sites that they appear between two to six weeks?

2- Is it possible that the hiv combo test did not pick up the antigine on day 23 and on day 26 my symptoms started due to the antibodies?and what about for hiv2 ? 

3- When do ars hiv symptoms appear after infection? And when can I do a HIV test to deny that these symptoms are due to HIV?

Very confused because of this surprising temperature, which I still have so far. Knowing that I did a covid 19 test and it was negative

I went a little while ago to take a shower and noticed swelling on the right side of my penis and I feel pain when I press on it, and I feel pain when I press on the opening of the head of the penis, and yesterday I had a lot of frequent urination, and I thought that it was because of the heat and I expect that the pain I feel in the pubis is a gland Swollen lymph nodes due to this swelling in the side of the penis.
In fact, I do not know what is happening, doctor, knowing that I did all sexually transmitted diseases 3 days ago and all of them are negative. What do you think is the reason for this swelling, doctor, and is it the reason for my feeling of fever?

Knowing that my wife has been feeling itchy in the vagina for a long time and feels sore in this area

I'm sorry you to hear of your new symptoms and that you found it necessary to return. I'll try to help, but this forum does not attempt to provide direct medical care. The main answers to your symptoms and related concerns will have to come from your own doctors.

1,2. As we have discussed previously, a negative HIV AgAb test at 23 days is too soon to be 100% certain about whether you caught HIV. However, that result is strong evidence that something other than HIV is the cause of your fever and other symptoms. I have never heard of anyone with negative results at 23 days in whom antibody first appeared or in whom symptoms appeared just three days later. And your other symptoms, i.e. genital area discomfort, are not typical for HIV or any other STI.

However, it would make sense to have a syphilis blood test, if not already done: it's still early for a positive result, and your genital area symptoms are not typical for syphilis. On the other hand, that's the only STI that comes to mind that might explain some if not all your symptoms. In addition, you should be carefully examined by a doctor who has experience with genital area infections:  an infectious diseases specialist or urologist would make sense. In the meantime, this forum does not attempt to provide direct medical care or advice about actual causes. You'll need to keep working with your doctors about what's going on. There are plenty of causes of fever other than HIV and COVID.

3. The symptoms of acute retroviral infection (ARS, i.e. new HIV infection) typically start 1-2 weeks after exposure. Three weeks is very atypical and that timing argues against HIV as the cause.

This forum is limited to discussion of STIs and HIV, and these remain very unlikely. Despite the timing, I believe it unlikely that your symptoms or your wife's have anything to do with the sexual contact described in your earlier threads. The most common cause of genital itching in women is vaginal yeast infection, but that would not cause your symptoms and most likely your symptoms and hers do not have a common cause. Today I went to the lab to get tested for sexually transmitted diseases and today is the 28th day of unprotected sexual intercourse and the results are as follows: hiv combo: non reactive
HBS AG test, result is non-reactive, HCS AB virus test, result: non-reactive, and RPR test: result: non-reactive
My last question on this topic is:
1- Do these results on the 28th day after sexual intercourse and two days after my symptoms, which are heat and pain, categorically deny that I have these diseases, or should I wait for a while after symptoms appear and do the analysis?
2- As you previously indicated to me that a hiv combo analysis after 4 weeks denies infection by 95%, am I now considered safe from hiv, hbs and RPR?

I hope you're getting professional care in person and not relying only on self-directed testing. Your fever and groin pain need professional evaluation.

1. The HIV result is around 98% conclusive. A negative HIV AgAb ("combo") test will be truly conclusive at 6 weeks. However, as already discussed, this negative result confirms that your symptoms are not due to HIV. The negative HBsAg, HCsAb and RPR tests are similarly strong evidence you didn't catch hepatitis B or C or syphilis, respectively, but those also need to be repeated at 6 weeks to be completely certain.

2. Correct, except it's more like 98% certain (not 95%). But see my comments above.

That concludes this thread. It should be your last question about this exposure and your symptoms and testing. Thanks for your understanding. But of course feel free to return once more if any tests or professional examination find that you indeed have an STD of any kind.
