[Question #8745] Herpes from Handjob by transexual sex worker

Hi,

4 weeks ago I received an handjob by a brazilian transsexual sex worker, she first masturbaded  her dick for 2 minutes and then, just after that, she gave me a handjob for approximatively 5 minutes, with the same hand, until I come; none fluid were involved during handjobs.

2 Weeks later I starting to feel some burning on my scrotum and the body of my penis (nothing on glans), this burning sensations last from 2 weeks now (not all the day but I feel it several hours a day, and my skin is highly sensitive when I touch it, becoming somewhat painfull), and the skin of my scrotum and penis is highly red colored. I didn’t notice anything else on my skin (sore, blisters, bumps or vesicles). But I have read than these buttons not always come when contaminated by herpes.

My main concern is thus that I have been contamined by HSV2. Maybe atypical form, or a slightly one? Not seen a doctor yet, waiting for my appointment. Can’t these symptoms be caused by hsv 2?

 I read on many officials websites about STD  (i.e the nhs in UK : https://www.nhs.uk/conditions/genital-herpes/) that hsv2 can be propagated by hands and fingers, especially when handjob is done after the person giving the handjob has rubbed his genitals before it. This is what I experienced. But I have read many comments from you (Asktheexperts) saying this is no possible to give HSV2 by mutual masturbation (handjob).

So where is the truth, and did I really took no risk of getting HSV2 with this transsexual ?

I’m very confused and worried.

Quick update.

I visited my doctor yesterday . He checked my genitals and groin. He saw nothing strange apart a swollen lymph node in the groin. He prescribed me a skin cream for my red irritated skin.

Seen the experience I had and the lymph node, he was mostly concerned about shyphilis. He said that this disesase is higly present among shemale sex workers population and that was the biggest risk I took with my encounter.

Despite the lymph node, he saw no sore but said it was maybe too soon to appear (can take up to 90 days) or maybe it could be a decapitated syphilis, a rare syphilis without sore. Seen as I’m constantly under azithromycin treatment (250mg/day) for many years now to cure lung infection, it could have be the cause of a decapitated syphilis.

He told me to come back in 2 weeks to make a blood test because it’s reliable only 6 weeks after exposure. And told me not to have unprotected sex with my wife until results.

I told him that experts on askexpertnow say it’s not possible to have STD’s transmitted by hands. But he was not convinced at all. He said, in my case, it was totally possible. So, Who is right?? Him or you ?? I’m totally confused and worried.

Can my daily intake of azithromycyn can play on the results of blood test in two weeks? I ve read that it can take up to 12 weeks to be 100% sure about syphilis with a blood test.

Did I really take a risk with syphilis? Must I suspend my unprotected intercourses with my wife until results (so maybe in eight weeks from now) ??

There is really nothing in your post that makes me concerned that you have herpes.  We don't believe that hands are a method of transmitting herpes, and you've had no sores.  IF you were going to develop sores with herpes, that would happen within 2-10 days, not 90, with a new infection.  I don't think there is a danger here that you have herpes.  I also don't believe that syphilis is a risk in this situation either.  I am dropping a note to our other physicians on this board who answer questions about syphilis to see if they have a concern.  I am not familiar with the term decapitated syphilis.  I understand that you are worried and confused, but I just don't think that your experience puts you at risk for these two sexually transmitted infections.

I have to add that during the 2 weeks after the exposure with this shemale sex worker, i was taking 1g of amoxicillin 3 times a day (so 3g/day) in addition of the 250  mg/day of azithromycin for my lung infection.

1) Can these intake of drugs have played a role on a possible syphilis? Could this have caused a decapitated syphilis and can i still be infected ? 

2)does it play a role on the delay of blood test to be sure that i have not syphilis ? in other words :is a 6 weeks delay sufficient or do i need to wait 12 weeks for a blood test?

I checked in with Dr. Hook and he also has zero concern about syphilis.  You do not need further syphilis testing.
