[Question #8780] Oral sex and symptoms

Hey Doctor,
Hope you don’t mind my question. I have recently found this site via mad googling and if I’m right for my money I get to speak to an actual doctor? That knows a thing or two about STI?
I’m just needing some guidance is all.
A week ago, I received oral sex from another male. I have done this in the past and I know the risks involved with this so whenever I get the urge to meet men I hold off sex with my wife for a week then test and if negative I get back to my wife.
I have over complicated it this time and even worse I have symptoms. I realise I have done this wrong but here we go.
So like a said a week ago I got oral sex. The day after I woke up needing a wee, I urinated and straight after I felt like I still needed to go. 10 mins later I urinated again, same thing. I was going every 10-20 mins and never had relief. Eventually i stopped going but I ALWAYS felt like I needed to go.
Panicked I booked a video app with a doctor who wasn’t so concerned and gave me a week of doxycycline 100mg twice a day.
I was taking this for a few days with no improvement so I ordered a STI test which I took on the 5th day of doxycycline 6th day after contact.
It’s negative for everything I have just got the results back.
Would the doxycycline have affected my results or am I safe to resume sex with wife? My symptoms have finally started to subside somewhat.
Thanks for your assessment.

Welcome to our forum. You are correct. You are receiving this Individualized reply from a physician who has specialized in diagnosis and management of sexually transmitted infections for many years.
Receipt of oral sex, While lower risk than exposures to infected partners through vaginal or anal intercourse, can lead to acquisition of infection. The most common infections to occur following receipt of oral sex are acquisition of gonorrhea or, very rarely chlamydia, or development of a process called non gonococcal urethritis (NGU) which results from introduction of a partner’s normal mouth bacteria into the penis to cause irritation. Unlike gonorrhea and chlamydia, NGU following receipt of oral sex is typically not transmissible to other partners. The most common symptoms of STI‘s acquired through receipt of oral sex are mild burning on urination or, particularly with gonorrhea, a urethral (penile) discharge. Urinary urgency and frequency of the sort you describe is not typically associated with STI‘s when this is the only symptom present. Urinary frequency and urgency may be symptoms of other non-STI processes including kidney stones, prostate infections (which are not due to STI), and anxiety.
There is no evidence that you have an STI. Currently available tests for STI‘s are amongst the most sensitive tests in all of the medicine. These tests can become negative when persons have taken antibiotics before they test such as you have. There is a slight possibility that you had non chlamydial NGU following your exposure which was successfully treated with the doxycycline. Irrespective, you should believe your test results. You do not have the most common STI’s, gonorrhea or chlamydia, and those are not at risk for infecting your wife. If you were symptoms continue, I would suggest to see a healthcare provider who can examine you add take additional specimens if necessary.
I hope this information is helpful to you. If any part of my response is unclear or there are additional questions you may use your ability to follow ups for clarification. Take care. EWH

Thanks for the reply doctor, I am a little confused so I’ll take your offer of a follow up if ok. (I’m probably been thick)
Firstly, symptoms, you don’t think they suggest an STI. The website that i found this through states it does
But I think what you are saying is in isolation it dosn’t suggest an STI?
Secondly, are you saying it is safe or not safe to continue sex with my wife? My symptoms have pretty much gone since my negative test (maybe anxiety like you said) or do I need to see a doctor for more tests first?
In regards to chlamydia or gonorrhoea you think I 100% don’t have them now? As in could the antibiotics have suppressed a positive result but not cured the infection?
Finally what are my risks in regards to STI then? Or do I just forget about STI and look for other causes or if I stay symptomless move on ?

Thanks for your follow up. The web site is incorrect regarding urinary frequency. My own research, which is the only research to look at this topic shows that urinary frequency is not associated with penile STI’s. I will notify the web master and have it changed.
Regarding your second question, yes it is safe to resume unprotected sex with your wife. Your test results are accurate. If there was an STI present which appears to be unlikely, the test proves that it is no longer present. Your treatment would not have suppressed the infection but if an infection were present might have cured it. Have confidence in your test results.
My advice of this time is to put your concerns about STI’s aside and move forward without concern. EWH
