I was diagnosed with genital herpes when I had a bad yeast infection. I’ve taken valtrex regularly (either daily for a period or as needed) for the last decade. I recently asked for a blood test and my igG came back negative (<0.91). Is it possible to have HSV-2 if igG is negative and should I get tested again?

Some additional information, I was able to get my past test from a decade ago. The doctor ordered a Cytomegalovirus (CMV) and HSV 1&2 igM.
My HSV 1&2 igG results (done in April 2022) are negative but the igM is slightly elevated. I recently had COVID. I had blood work done about 10 days after a positive test and after testing negative (at home test) and all my symptoms were resolved.

How were you originally diagnosed - blood antibody test, swab test?
You've taken Valtrex daily the entire time, is that correct? Have you had any recurrences during that time?

My original test was from bloodwork. They did not do a swab from the records they provided me. I’m not sure why my old dr ran the CMV test. I don’t have HIV or a compromised immune system.
Old labs (2007)
CMV igG 1.0
CMV igM 0.9<
HSV 1&2 igM 1.63
New labs (2022)
HSV1 igG <0.91
HSV2 igG <0.91
HSV 1&2 igM 1.04
I took valtrex daily for a year when first diagnosed, then it shifted to as needed. I had not been retested until my recent annual. I’ve never had an outbreak but I’ve taken valtrex anytime I felt an itch that I thought might be something. I’ve never had lesions or blisters.
Should I get tested again?
If I had herpes, would valtrex lower my antibodies to undetectable?

You've never had a reliable test to say that you have herpes. The CDC says never to do use it to try to diagnose HSV. And your current test is negative. The IgG test misses 30% of HSV 1 infections and 8% of HSV 2 infections, compared to the gold standard western blot. The Valtrex question: Have you had at least three months off of Valtrex in the past decade? If yes, then your body has had an opportunity to make antibody off drug.

Hi Terri - I really appreciate your advice and insights. After my results, my PCP and I discussed discontinuing valtrex and retesting in 3 months.
Do you think that is a good plan? I have had a lot of anxiety about having herpes and I don’t want to drive myself into a bad place.
I’m also trying to communicate what’s going on with my spouse. They have remained herpes free since my diagnosis— we have sex without condoms.

I think it is fine to discontinue your Valtrex, absolutely. Remember if you are truly infected, that would make any sex partners more vulnerable to becoming infected. But have you been off Valtrex for a 3 month period prior to this in the past 10 years? If yes, then the testing should be accurate now. But your call. If you need further help, come back and post again. I'm guessing your visual diagnosis may have been incorrect. Or if you were diagnosed by IgM test alone, that was a very significant error.
