[Question #8822] Hiv Risk after Oral Sex

Hello, I asked my question using a last reply of another question, but I thought it wasn’t fair, after all the good advise and effort put on this page. 

I had unprotected oral sex with another male yesterday night, where I gave and received. There wasn’t ejaculation and I don’t have any cold sore/ulcer or cut in my mouth, or at least not noticeable. According to a doctor, there is no need of pep, since it is a almost negligible risk, and the cdc says the risk is “low”. But I’m a little anxious and worried. 

You guys are always been my source of truth so I wanted to ask you if it is ok to say I’m safe from that exposure and it is extremely unlikely to catch hiv from it. 

Regarding the other STD, I took 200mg of doxycycline as a “PEP”, since I read it could be helpful in preventing them. 

Also, I wanted to ask you regarding the testing, if I take a RNA test at day 11 is it conclusive? Or should I just wait for the 28 days+ of the 4th gen?

Welcome back to the forum. Thanks for your sense of fair plate. And that you have asked several additional questions in this new question, I will go on and address them, Expanding on my comments about just with a low risk exposure this was.

I believe I have addressed your questions regarding HIV other than the need for testing. As already mentioned in our earlier exchange, your risk for acquisition of HIV from the exposure you describe is vanishing low and should not be a concern. If you feel the need to test to rule out HIV (personally, in your situation, I would not) related to this exposure an HIVRNA test taken at 12 to 14 days will provide very, very strong evidence that you were not infected. While many experts, including ourselves on this forum, suggest that persons follow up a 12 to 14 day negative HIVRNA test with a standard fourth generation test at six weeks, given the nature of your exposure (very low) and the fact that I have never seen or heard of a person with a - HIVRNA test who then went on to become positive, In your situation would consider the RNA test conclusive and not feel the need to repeat it at a later date. 

Regarding other STI’s, the major STI transmitted through oral sex is gonorrhea. To be confident that you were not infected I would suggest a urine test for gonorrhea as well as a throat swab for gonorrhea performed at any time more than three days after your exposure. Most Gonorrhea tests are combined with chlamydia test so you will get a Chlamydia result as well although chlamydial infection are far less common at the throat than gonorrhea.  The doxycycline prophylaxis that you took will eliminate the possibility of Syphilis, chlamydia, and Nonchlamydial NGU but it will not necessarily prevent Gonorrhea.

I hope this additional information is helpful. Once again let me emphasize that this was a very, very low risk exposure. Please don’t worry. EWH

Thank you for the fast answer dr. I think it will be ok to wait a little bit longer and use the 4th gen (1/3 of the price). At 4 weeks is 95% accurate right? 
Regarding the chlamydia/gonorrhea test I saw a home kit with the urine and swab. Is that ok? Also, I took only one doxycycline set, do I need to take during the following days or with those 200mg is ok? 

Thank you again, now I will need to reduce my stress and think that is is highly unlikely a hiv infection

Actually I would estimate the accuracy of a 4th generation combination HIV antigen/antibody test at greater than 98% as a generalization.  Given the nature of your exposure , there is little downside to your plan.  I am confident that when you test you will find that you did not acquire HIV from the exposure you described.

Yes antibiotic PEP for prevention of NGU, chlamydia and syphilis I would take 200 mg of doxycycline daily for five days.

Thank you doctor, as always.
My last question is a more detailed version of the first question, because I was embarrassed of my actions. The exposure itself was in a bathhouse, which is not the best place for hygiene. It wasn't my best idea being honest, and as I said, I didn't engaged in multiple partner exposures or something like that. It was something really unexpected of me and when I got my conscience back, I stopped the exposure and left the place, but I think is good to ask if that scenario affects the previous responses, due to the place which could be of higher risk.

I think it wouldn't make such a big difference but it is better to ask.

Adding up to the final question: 

Do you consider safe if I continue my normal sexual life with my girlfriend? That is one of the things that is making me more anxious. You are telling me it is a no risk exposure so everything should be fine right? Or do you think it is better to wait until the test at 3+ weeks.

Sorry for the delay in responding. The location of your encounter makes no difference in your risk of acquisition of any STI including HIV, this was a very, very low risk encounter.  STI’s are not transmitted through contact with any contaminated inanimate objects. 

As far as continuing unprotected sex with your regular partner, we’re I in your position, I would have no hesitation and no concerns about having unprotected sexual contact with my regular partner.

Hello Dr. Sorry, I know the 3 questions were already made. I have no problem buying another set. Is it ok if I take the RNA at day 10/11? You told me at day 12-14 but I got a job related trip and I'm planning on taking it tomorrow. Is it still accurate?
