Hi Doctors,
I'm living in Thailand, I did 2 stupid things please help me assess the risks.
Last month I did something stupid 2 times, I think I lost my logical thinking for a moment from stresses.

1st Incident,
1st April 2022, I had a protected sex with CSW, it lasted about 10 mins and I think condom was intact thoughtout the session. After that I got tested 4th generation test(CMIA) on 21st April 2022, and on 28th April 2022 after the potential exposure, all of them came back negative. Could I move on from this event with consideration of 2 existing tests and risk?

2nd Incident,
22th April 2022, I visted a pretty spa in Bangkok. She gave me a protected oral sex, however her saliva was on a condom and she also poured some amount of lubricant on that too(It might contain some chemical substances, I felt cool around the condom). She used her breast to masturbate me so her saliva was presented on her breast after that about 4-5 mins(not immediately) I suck her nipples and taste some of saliva(I just sucked nipples not entirely breast) I might have had mouth ulcer or bleeding gum on that day, For this event I haven't got tested, please help me. Do I need get tested for the 2nd event?

I appreciate your advice doctors.

Welcome to the Forum.  Thanks for your questions.  I'll be glad to comment.

1rst encounter.  Condom protected sex is safe sex as long as the condom is worn throughout the encounter and does not break (when condoms break, they break wide open.  If your condom appeared intact, it was) .Thus this was a virtually no risk encounter.  The results of your follow-up test on April 28 confirm this as results of HIV testing with a 4th generation test taken at 28 days are more than 98% conclusive (and personally, I have never seen or heard of a test which was negative at 28 days subsequently becoming positive.  Thus, although official recommendations are that results are not conclusive until 6 weeks following exposure, I am confident that you did not get HIV from this encounter.

2nd encounter.  HIV is not transmitted in saliva and there are no proven instances in which someone has acquired HIV from receipt of oral sex.  Similarly, receipt of masturbation of any sort, even with saliva as a lubricant is a no risk exposure.  HIV uis also no acquired as a result of sucking on an infected partner's breast.  There is no medical or scientific need for testing related to this encounter.

I hope that this information is helpful.  If there are further questions or any part of this reply is unclear, please use your up to two follow-ups for clarification.  Take care.  EWH

Dear Dr.Hook

For the 1st encounter,  your answer is clear It seems I can trust 28 days test result.

For the 2nd encouner, I have some doubts even I had cuts or bleeding gum I might have on that day.
Is it considered to be a risk?  Like I said Dr.Hook I sucked her nippled, and tasted an amount of saliva(it might be a mixture of saliva and chemical lubricant), could  it be considered I sucked contaminated objected which was exposed to the air for about 4-5 mins? and how risk it is, if I have cuts and bleeding gum on that day? please help me clarify this

Thank you Dr.Hoook

Cuts and scapes would not change things, even sores in your mouth.  Still no risk.  EWH ---

HI Dr.Hook,

Just a minor concern, April 28 is considered to be 27 or 28 days from the exposure. And does it significantly affect test reliability(98%)? Because I faced the exposure on April 1, and how medical date is counted 27 or 28 days for my case.

We provide up to three responses to each client’s questions.  This thread should have been closed.  Final response.

The one day difference makes no difference following your very, very low risk exposure.  Have confidence in your results.  EWH
