[Question #8853] dumb mistake

Dr Handsfield since my last question I have been extremely diligent to practice safe sex. however 14 days ago I met up with a cow who I found online.  On her ad she states everything must include a condom no exceptions.  Upon arrival she realized she had left her condoms at her hotel and stupidly in a drunk state of mind I said that was ok.  I engaged in unprotected oral followed by 5 minutes of unprotected vaginal.  She assured me she wad clean and never did this. With that said in the last 14 days I didn't have any symptoms that were noticeable minus sensations like tingling feeling the urge to urinate more than often and feeling a slight tingling but no discharge.  this morning I woke up and after urinating saw a brief clear sticky discharge come out.  I have been in contact with the csw who has assured me she was tested recently and she is fine.  I havnt had a chance to test for anything yet but for sake of argument does this sound like chlamydia.  If it is what do u think my chances of HIV are.  it's been 14 days and I havnt had a fever sore throat or body rash.  I've looked at all of her post that all state nothing uncovered and just to test it had a friend message her about bare service and I messaged her about repeat bare service to which she replied condom only.  I am freaking out and worried as I await the 4 week mark to test for everything.  we are in Los Angeles she is AA 30 years old according to her post.  please advise as I could us some reassurance while I wait.

Welcome back to the forum. Congratulations to your commitment to safe sex. Occasional slip ups do occur so I hope you won’t beat yourself up on this. 

The encounter you described was low risk, even if your CSW partner was infected or if she was not truthful about regular condom use.  Let’s review the facts regarding your exposure.

1.  Most commercial sex workers do not have STI’s and fewer still have HIV.
2.  Even in the unlikely circumstance that your partner was infected, most exposures do not result in transmission of infection. HIV is transmitted through unprotected genital sex less than once in every 1000 encounters and it’s not known to be ever transmitted through receipt of oral sex. Other STI‘s such as gonorrhea and chlamydia are more easily transmitted but still the likelihood of acquisition of infection is quite low.
3.  Perhaps less importantly, the symptoms you described are not associated with STI’s. You do not describe whether you the urinary tingling occurs during urination or not. If it does not occur during urination it is unlikely to be associated with an STI. Similarly, our own research indicates that urinary frequency is not a symptom of STI’s.

Bottom line, There is a greater than 99.9% chance that you did not acquire HIV through the exposure that you describe. While your risk for STI‘s such as gonorrhea and chlamydia is somewhat higher, it is still quite low. Given your concerns and anxiety however, I would encourage you to get tested for Gonorrhea and Chlamydia with a urine test. The tests are quite accurate and easily to obtain confidential confidentially.
