1.  Regarding your eight week test for HIV, please have complete confidence in the results. The results of tests for HIV taken eight weeks after an exposure are completely conclusive and should be believed. There is no need for additional testing.

2.  You are correct, that transmission of genital herpes due to HSV-1 to partners is far less common than transmission of genital herpes due to HSV-2.  Nearly all genital herpes following receipt of oral sex with a partner who has a cold sore is due to HSV-1.

3.  Any lesion or sore occurring just six hours after a sexual encounter is unlikely to be due to an STI.  Who is in you describe is unlikely to be due to any STI.

We provide up to three responses to each clients questions.  This is my third response. Soon after this response the thread will be closed and there will be no further responses.  In answer to your follow-up questions:

1.  Your 59 day HIV tests are conclusive and prove that you did not require HIV from the exposure you have described. There is no need for further concerns about HIV.

2.  Determining which type of herpes using blood tests may have caused your herpes is difficult.  The blood test for HSV-1 ( herpes simplex type-1) misses many diagnoses and can be positive in persons who have cold sores or oral herpes that they acquired in childhood. Thus, neither a negative or a positive blood test for HSV-1  is helpful. On the other hand, if you did not receive oral sex during your encounter with the commercial sex worker, you can be confident that you were genital lesions we’re not due to HSV-1 although they may have been due to HSV-2.  

Blood tests for HSV –2 perform better that blood tests for HSV-1.  Blood test for HSV-2 may take up to six months to become positive however and even then as many as 10% of persons with genital herpes due to HSV-2 may not have positive blood tests.  

If your genital lesions were due to herpes and caused by HSV-1 there is a little or no risk that you will sexually transmit this infection to your wife. If your lesions were due to HSV-2, on average 3 to 5$ of regular sexual partners acquire infection from their infected partners each year. Taking antiviral medication such as acyclovir can reduce the likelihood of transmission of herpes by about 50%.  As this series of answers to your questions is about over, if you have further questions about herpes you should start a new thread which will be answered by our herpes Expert, Terri Warren.

3.   I see no reason for additional testing for other STI’s.
